Renal outcomes according to renal replacement therapy modality and treatment protocol in the ATN and RENAL trials

Abstract: Background In critically ill patients with acute kidney injury, renal replacement therapy (RRT) modality and treatment protocols may affect kidney recovery. This study explored whether RRT modality and treatment protocol affected RRT dependence in the ‘Randomized Evaluation of Normal versus Augmented Level of RRT’ and the ‘Acute Renal Failure Trial Network’ (ATN) trials. Methods Primary outcome was 28-day RRT dependence. Secondary outcomes include… Show more

“…Another recent secondary analysis pooled individual patient data from the RENAL and ATN trials to compare kidney outcomes by initial KRT modality. 83 Of 2542 patients included, 2175 (85.5%) initially received CKRT and 367 (14.4%) received intermittent HD. CKRT patients had higher illness acuity and were receiving more organ support at randomization.…”

“…The trial drug was provided in addition to usual care as outlined in the Surviving Sepsis Campaign guidelines [25] and Kidney Disease Improving Global Outcomes (KDIGO) guidelines [6]. Initiation and termination of RRT were based on conventional criteria [26][27][28]. A Data Management Committee (DMC) was installed and provided with stopping rules based on predefined threshold for futility or early success.…”

“…Despite the growing clinical usage, patient outcomes with KRT modalities remain controversial [ 6 ]. Several clinical trials [ 8 , 9 ] and meta-analytic studies [ 10 13 ] demonstrate no significant differences in in-hospital mortality, in-ICU mortality, renal recovery, or dialysis dependence between KRT modalities. However, these studies exclusively recruited patients with AKI.…”
