Aims/Introduction: Irisin is a newly identified myokine which can promote energy expenditure. Urotensin II (UII) is identified as the most potent mammalian vasoconstrictor to date. Previous studies showed that UII can aggravate insulin resistance while irisin alleviate insulin resistance. Through this study, it is our aim to elucidate if UII can induce insulin resistance and also have an association with the irisin level in hemodialysis (HD) patients. Materials and Methods: One hundred and twenty-eight patients on maintenance hemodialysis treatment and forty healthy subjects were enrolled in this study. Blood irisin concentrations and UII concentrations were measured by ELISA and RIA respectively. The body composition was analyzed by bioelectrical impedance. Results: The serum irisin levels and UII levels were both significantly lower in HD patients in comparison to that of the healthy subjects. The serum irisin levels were lower in HD patients with protein energy wasting than those of the patients without protein energy wasting. The independent determinants of circulating Ln (irisin) (the natural logarithm of irisin) were UII lean body mass and patients with protein energy wasting. Conclusions: Our results are the first to provide the clinical evidence of the association among irisin, UII, and protein energy wasting. Our results hint that UII and protein energy wasting might inhibit the release or synthesis of irisin from skeletal muscles in HD patients.

Background/aim: The pathogenesis of Raynaud's phenomenon (RP) has not yet been fully elucidated. RP is characterized by exaggerated cold-induced vasoconstriction. Urotensin II (UII) is a potent vasoconstrictor. The aim of the present study was to evaluate plasma UII levels in both primary RP and secondary RP associated with systemic sclerosis (SSc). Materials and methods:Fifteen patients with primary RP, 30 patients with RP secondary to SSc, and 30 healthy controls (HC) were included in the study. Raynaud condition scores (RCS) were determined in the primary RP and SSc groups. Modified Rodnan skin score (MRSS) was determined for the SSc patients. Plasma UII level was analyzed by the ELISA method.Results: When compared to the HC group, plasma UII level was lower in the secondary RP group, but not in the primary RP group. Plasma UII level was not directly related to RCS in either the primary or secondary RP group. Moreover, it was not correlated with MRSS in the secondary RP group. Conclusion:The results of the present study suggest that UII is not associated with primary RP. Its level was lower in the secondary RP (SSc) patients. Therefore, it can be concluded that decreased UII level is related to SSc instead of RP.

ÖzetBir siklik petid olan Ürotensin 2 (ÜT 2) yaklaşık 45 yıl önce keşfedilmiştir ve güçlü vazokonstriktör etkilerinin olmasından dolayı bilim camiası tarafından dikkat çekmiştir. ÜT 2 G proteinine bağlı bir reseptör olan "GPR14" reseptö-rüne bağlanarak etki gösterir. ÜT 2 ve reseptörü yaygın olarak periferik vasküler dokularda, kalp ve böbrekte dağılım göstermektedir. ÜT 2'nin sağlıklı insanda çok küçük bir role sahip olduğu belirtilmiş, hastalık durumlarında ise nispeten rolünün daha belirgin olduğuna değinilmiştir. ÜT 2'nin kardiyovaskü-ler etkileri insanda başlıca pozitif inotropi, vazokontsriksiyon ve vasküler yatağın ve endotelin durumuna bağlı vazodilatasyon ve vasküler düz kas hücre proliferasyonudur. Günümüzdeki araştırmalar ÜT 2'nin özellikle çeşitli kardiyovasküler hastalıklarda önemli bir rol oynadığını göstermiştir. ÜT 2 ve dolayısıyla ÜT 2 reseptörlerinin kardiyovasküler sistem üzerindeki etkileri günü-müzde oldukça çok çalışılan bir konu olmakla beraber literatürde bu konuyla ilgili çalışmalar ve bu çalışmalar sonucu elde edilen sonuçlar oldukça karı-şıktır. Kalp yetmezliğinde daha önceki çalışmalarda majör bir rol oynamadığı söylenmiş, daha sonraki çalışmalarda hastalığın patofizyolojisinde potansiyel role sahip olduğu gösterilmiştir. Ayrıca koroner kalp hastalığı ve iskemik kalp hastalığı ve aterosklerozda da ÜT 2 seviyesinin arttığı gösterilmiştir. Bu reseptörlerin fizyopatolojide mi rol oynadığı veya çok önemli bir savunma makanizması mı olduğu önemli bir araştırma konusu olmaktadır. Bu nedenle bu reseptörler bilim camiasında umut verici bir hedef olarak değerlendirilmekte-dir. Sonuç olarak bu reseptörlerin ileride çoğu hastalıkta özellikle kardiyovasküler hastalıkların gerek patofizyolojisi gerekse savunma mekanizmalarında kendilerine yer edineceğini ve daha fazla önem kazanacağını düşünmekteyiz. UT 2 binds to a class of G protein-coupled receptor known as GPR14. Receptors for UT 2 are located on peripheral vascular smooth muscle, heart and kidney. In healthy people it has minor role, while in patients with disease it has a more prominent role. Main positive effects of UT 2 in cardiovasculer system are positive inotropic effect, vasoconsctruction, vascular smooth muscle cell proliferation, vasodilatation which related the state of endothelin and vascular bed. Recent investigations have showed that UT 2 plays an important role in cardiovascular diseases. Although the effects of UT 2 and UT 2 receptors on cardiovascular system is a quite extensively studied subject. The studies and the results obtained from these studies are somewhat complex. Abstract: These data suggest a potential vasodilatory role of UII in humans with kidney disease or hypertension. The reduction in UII levels in CKD also suggests either reduced production or greater clearance, or both, of UII.
