“…This is consistent with the prior literature which showed that at progressive more severe degrees of CKD, tubular secretion of creatinine plays an increasing prominent role in clearance [3,28,31-33]. We interpreted the observation that CKD patients with higher BUN, high creatinine and lower hemoglobin had higher CrCl/iGFR ratio as being because higher BUN, high creatinine and lower hemoglobin reflect lower GFR values.…”

“…This finding is independent of the creatinine assay used (legacy vs IDMS) because the measured Cl cr calculation requires both urine (numerator) and serum (denominator) to be analyzed using the same assay. Because creatinine is both filtered and secreted in the tubule, Cl cr should be 10–20% higher than true GFR . Although true GFR was not measured in the BLSA study, it is very likely that the MDRD and CKD‐EPI equations would have overestimated true GFR in our population because these equations exceeded both measured and estimated Cl cr .…”

“…If the liver, which is the main organ for detoxification of all chemical compounds, is injured, hepatocellular enzymes such as AST and ALT are released and are found in high concentrations in the blood. Finally, BUN and creatinine are the clinical markers of renal dysfunction, for example, renal obstruction or damage 42. If these values were elevated at the start of the protocol, subjects would have been excluded.…”
