Renal function overestimation in underweight and/or non-ambulatory patients

Abstract: Creatinine clearance estimation is widely used to evaluate the renal function of the patients in order to initiate or adjust the drugs dosage. However serum creatinine, as a muscle metabolism by-product, may not be a reliable parameter in underweight and/or non-ambulatory patients, such as geriatric, acquired immunodeficiency syndrome patients and bed-confined and cachexic cases. To avoid overestimation of the renal function in those patients, serum cystatin C can be considered as a sensitive and accurate alte… Show more

“…Because of the small sample size expected for proteinuria, we only report descriptive variables and frequencies. In sensitivity analyses, we reanalysed the predictors of eGFR and CKD by ancestry; excluded persons with underweight (BMI < 18.5 kg/m 2 ) because of potential low creatinine with overestimation of the kidney function [26]; and reanalysed the data estimating GFR ≤ 60 mL/min/1.73 m 2 with the CKD-EPI equation. Outcomes in parentheses are mean (SE) and in square brackets are 95% confidence intervals unless stated otherwise.…”

“…In this condition, enalapril and other ACE-inhibitors conserve kidney tissue by lowering intraglomerular pressure [5] and estrogen can also play a beneficial role. In practice, renal status is usually monitored via estimated glomerular filtration rate (eGFR) derived from serum creatinine clearance, but this is less reliable in underweight conditions where serum cystatin C is preferred [6] . Here an unusual alignment of RUNX2, SALL1, and SAMD9 variants is presented in the context of additional intron mutations, where pediatric Covid-19 illness and renal injury emerged in the setting of early ovarian insufficiency.…”
