Renal Cell Carcinoma With Inferior Vena Cava Tumor Thrombus in Two Patients With Previous Coronary Artery Bypass Graft: Strategy for Surgical Removal

Abstract: Surgical management of renal cell carcinoma (RCC) with tumor thrombus (TT) extending into the inferior vena cava (IVC) and up to the hepatic veins and right atrium (RA) continues to be problematic and a challenging surgical operation. It becomes even more complicated when performing a re-sternotomy and cardiopulmonary bypass (CPB) in patients with previous coronary artery bypass grafting (CABG). Here, we report on two patients with previous CABG who presented with RCC and TT extending into the hepatic vein and… Show more

“…This is in accordance with previous studies demonstrating similar mortality rates in the mild-term follow-up. [29][30][31] In summary, this is a retrospective study with a very small sample size which precludes any confirmatory conclusions to be derived from our results. However, the data show that with both strategies, hypothermia and normothermic CPB during extraction of stage III-IV RCC, a safe perioperative course can be realized.…”

“…Our concept of inferior vena cava reconstruction is to use as few artificial prostheses as possible. If prosthetic replacement is necessary, we try to reconstruct the inferior vena cava with a patch as much as possible [ 9 ]. In such cases, we do not use the patch itself, but cut a portion of the tube graft to make it easier to follow the native vessel.…”

“…Even if a thrombus is located above the diaphragm, a method to perform thrombectomy without performing sternotomy or CPB has also been introduced [ 37 - 39 ]. For this procedure, it is necessary to first completely isolate the IVC from the central tendon of the diaphragm.…”
