Removal of Broken Long Gamma Nail: A Modified Guide Wires Technique

Abstract: We report a simple alternative procedure, modified ball-tipped guide wires technique, to remove a broken long gamma nail at the level of lag screw hole which is detected during a removal procedure. The fragment retained in the medullary canal was successfully removed without complication.

“…Then the ball-tipped guide wire is pulled back to extract the nail [2, 3] (Figure 2). The end of the ball-tipped guide wire can be bent to increase its chance to pull the piece of the broken nail [4]. Karladani described a technique in which he used one guide wire that was trapped in the canal using a 3.5 mm screw inserted through the locking hole [5].…”

“…Additional techniques for removing broken intramedullary nails include using hooks [1], a Hohmann retractor through a working channel in the lateral femoral cortex similar to a shoehorn (called the push-out technique) [11], or Ender nails [13], which must be placed beyond the distal tip of a broken nail and then hooked through the locking holes of the nail [16]. Other techniques use sigmoidoscopy forceps [12], distally impacted multiple guide wires [14,17], or custom-made devices [6]. All these techniques were described for removal of the distal fragment of a broken nail, and none could be used in our situation with a jammed intact intramedullary nail.…”

“…Riansuwan et al have described an alternative procedure for extraction of broken gamma nail. With this method broken fragment inside the channel was extracted successfully and no complication occurred [19]. Steinberg et al was applied a method by which distal fragment of broken intramedullary nail inside channel can be extracted without requiring an additional approach to bone, without using any special equipment or device and without any damage to channel [40].…”
