Remodelamento ventricular na estimulação cardíaca apical do ventrículo direito

Abstract: Conclusion: In a long-term follow-up, patients without ventricular dysfunction who underwent RV apical pacing (RVAP) showed low VR rate, and no analyzed variable was associated with its occurrence.

“…The sample size may have been insufficient to detect subtle changes in ejection fraction. Silva et al 37 evaluated ventricular remodeling in patients with ventricular apical pacing. The remodeling was defined as echocardiographic changes documented for at least six months after implantation: > 10% increase in left ventricular diastolic diameter and > 20% decrease in ejection fraction.…”

“…The scientific evidence regarding indications for ICD in CCC is limited to publications of case series, retrospective cohorts or registries, involving only secondary prevention of cardiac SD [237][238][239][240][241][242][243][244][245][246][247][248][249] . So far, no large-scale randomized clinical trial, comparing the efficacy of ICD with active drug or placebo in CCC, has been published (Table 12).…”

“…The studies MOST 11 , DAVID 12 and MADIT II 13 , in turn, indicated that VD can create an anatomofunctional substrate capable of impairing heart function in the long term, by observing an increase in the risk of AF, mitral regurgitation and hospital admissions due to heart failure in patients with a high percentage of right ventricular paced beats, particularly those with ventricular dysfunction prior to the implant 14 . However, whether VD is an acute phenomenon that could deteriorate LV function in the long term and, consequently, result in HF in the absence of cardiomyopathy, remains to be clarified 15,16 .…”
