Remdesivir induced viral RNA and subgenomic RNA suppression, and evolution of viral variants in SARS-CoV-2 infected patients

Abstract: While changes in SARS-CoV-2 viral load over time have been documented, detailed information on the impact of remdesivir and how it might alter intra-host viral evolution is limited. Sequential viral loads and deep sequencing of SARS-CoV-2 recovered from the upper respiratory tract of hospitalised children revealed that remdesivir treatment suppressed viral RNA levels in one patient but not in a second infected with an identical strain. Evidence of drug resistance to explain this difference was not found. Reduc… Show more

“…Our data highlight that infection control measures may need to be tailored to the needs of immunocompromised patients and also caution in interpretation of CDC guidelines that recommend 20 days as the upper limit of infection prevention precautions in immune compromised patients who are afebrile 19 . Due to the difficulty with culturing clinical isolates, use of surrogates are warranted 20 . However, where detection of ongoing viral evolution is possible, this serves as a clear proxy for the existence of infectious virus.…”

“…The added benefit of examining whole genome is that linked mutations can be identified statistically using an approach that we gave recently developed 29 . Indeed haplotype reconstruction has proved beneficial in the analysis of compartmentalisation and diversification of several RNA viruses, including HIV-1, CMV and SARS-CoV-2 30,48,49 . Key findings of this study were: firstly that diversity increased over time with variable trajectory away from the consensus baseline sequence and also the reconstructed ancestral subtype M consensus.…”
