O presente estudo teve por objetivo determinar a eficácia, efetividade e segurança do Remdesivir no tratamento para a Covid-19 através de uma revisão integrativa. A busca dos artigos foi realizada em março de 2021 nas bases de dados Pubmed, Scielo, Embase e Cochrane. Para guiar essa pesquisa, levantou-se a questão norteadora: “Qual o papel do Remdesivir no tratamento da Covid-19 em humanos?”. Foram utilizados os descritores “Covid ou Sars-Cov-2 ou Coronavirus” e “treatment” e “Remdesivir”. Os critérios de inclusão foram estudos primários originais que abordassem o tema proposto, disponíveis na integra, entre 2019-2021, nos idiomas português ou inglês. Dos 3012 artigos encontrados, foram utilizados 17 que atenderam os critérios de inclusão. A maioria dos estudos encontrados relatou uma melhora no quadro dos pacientes tratados com Remdesivir, porém é importante ressaltar que a terapia com o medicamento também apresentou diversos efeitos colaterais e não alterou o índice de mortalidade. Com base nos dados analisados, o Remdesivir demonstrou resultados promissores, sugerindo um perfil de risco-benefício favorável, porém até onde se foi observado, a tava de óbitos não foi alterada e seu uso prolongado pode desencadear efeitos colaterais indesejáveis.

Objective to evaluate antiviral treatment, duration, and side effects on pregnant women based on gestational age and severity of COVID-19 infection. Method: a systematic review of antiviral treatment, duration, and side effects on pregnant women based on gestational age and severity of COVID-19 infection. Systematic review was conducted following the guidelines of the Preferred Reporting Items for Systematic Reviews and Meta-analysis (PRISMA) Statement. Result 948 papers accessed through Pubmed, Scopus, Science Direct, Cohcrane, and other with keywords “Antiviral”, “Pregnancy” “Pregnant” “Coronavirus” “COVID-19” “SARS-CoV-2”. Duplicate papers were excluded (n=302), topics and abstracts that do not meet the criteria (n=612), and 25 papers that did not meet the inclusion criteria. 9 papers that meet the inclusion criteria (case reports and cohort retrospective case study) discussed 20 pregnant women with COVID-19 infection (16 moderate and severe cases received Remdesivir, 3 moderate and mild cases received Lopinavir-ritonavir combination, and 1 moderate case received Arbidol). Conclusion, remdesivir is an antiviral frequently used in pregnancy on trimester II and III with severe COVID-19 infection with a duration of treatment of 5-10 days. Remdesivir should be monitored because some show side effects of increasing liver function. Key word: Antiviral, Pregnant, COVID-19

Coronavirus disease 2019 (COVID-19) has resulted in an unprecedented global healthcare crisis. One special population that poses a challenge is pregnant women with COVID-19. However, there is limited data on treatment options for severe coronavirus disease in pregnancy. Remdesivir, an antiviral drug, is currently being studied as a potential treatment of COVID-19 pneumonia. Nevertheless, pregnant women are also being excluded from various clinical trials for the disease. There are some studies mentioned in the literature which have shown no adverse effects of remdesivir during pregnancy. In this study, we present four serial cases of COVID-19 in pregnant women with moderate to severe symptoms who were treated with remdesivir. All of the them showed positive fetal outcome without any birth defects or malformations. However, further studies are necessary to evaluate the biosafety and effects of remdesivir in pregnant women.
