Reliable Determination of Fetal RhD Status by RHD Genotyping from Maternal Plasma

Abstract: Background: Immunoprophylaxis with IgG anti-D is a standard prevention of hemolytic disease of the fetus and newborn. Fetal Rhesus D (RhD) blood group genotyping from maternal plasma of RhD-negative pregnant women allows targeted prophylaxis with IgG anti-D in RhD-positive pregnancies only. We set up a reliable protocol for prenatal RHD genotyping. Methods: 153 pregnant Caucasian RhD-negative women were tested in the… Show more

“…In European countries, even if false-negative genotyping results are at very low rates [19,20], to further limit the risk, different strategies emerge. The attractive idea of using a foetal DNA marker in the foetal genotyping test is not so easy to apply [12].…”

“…Despite the abovementioned challenges, noninvasive fetal RHD typing is very robust and reliable, also in the first trimester, as a result of a large research endeavor that has generated technical advances and method refinement. Current assay performances are characterized by near 100% accuracies and sensitivities, surpassing a former assessed overall accuracy of 96.5% for detecting plasma cffDNA, based on a meta‐analysis of studies from 1998 to 2005 …”

“…Finning and colleagues applied a setup targeting exons 4, 5, 6, and 10, obtaining an accuracy of 100% ( n = 137) . Several assays have been reported with a similar setup, targeting several exons . For example, Minon and colleagues obtained an accuracy of 99.8% ( n = 563) .…”

“…Significant progress in prenatal care strategies for the fetus with RhD has occurred during the last few decades. Cell-free fetal DNA (cffDNA) discovered from plasma of pregnant women by Lo et al in 1997 has been used for the noninvasive detection of fetal RhD status [ 7 9 ], which has the potential to avoid antenatal anti-RhD prophylaxis in RhD negative women [ 5 , 10 12 ]. Many obstetricians accept fetal RhD status detection using circulating cffDNA from maternal plasma or serum.…”
