Reliability of ultrasound grading traditional score and new global OMERACT-EULAR score system (GLOESS): results from an inter- and intra-reading exercise by rheumatologists

Abstract: This study aims to test the reliability of ultrasound to graduate synovitis in static and video images, evaluating separately grayscale and power Doppler (PD), and combined. Thirteen trained rheumatologist ultrasonographers participated in two separate rounds reading 42 images, 15 static and 27 videos, of the 7-joint count [wrist, 2nd and 3rd metacarpophalangeal (MCP), 2nd and 3rd interphalangeal (IPP), 2nd and 5th metatarsophalangeal (MTP) joints]. The images were from six patients with rheumatoid arthritis, … Show more

“…By electing to reference visible portions of the patella and quadriceps tendon, we obviated the need for a specific probe size when acquiring images. Although videos have been used in some adult scoring studies , we were able to capture a wide area of synovitis and maximal Doppler signal using still images alone. This restriction also eliminated the challenge of interpreting movement artifact, and it increased scanning efficiency, because less time was required for image acquisition.…”

“…The study by Ventura-Rios et al [21] presented a different system of US assessment of joints than that presented in this paper. The system was presented by Outcome Measures in Rheumatoid Arthritis, Clinical Trials – European League Against Rheumatism (OMERACTEULAR) and regarded the assessment of synovitis.…”

“…Synovial hypertrophy and synovial vascularity were graded semi-quantitatively on a scale of 0 to 3 according to the OMERACT-EULAR task force. [5][6][7] Tenosynovitis and erosions were evaluated. Acro-osteolysis was defined as an abrupt ending of the hyperechoic cortex or complete disappearance of the cortical hyperechoic line of distal phalanx.…”
