Reliability of radioisotope-guided sentinel lymph node biopsy in penile cancer: verification in consideration of the European guidelines

Abstract: BackgroundLymph node (LN) staging in penile cancer has strong prognostic implications. This contrasts with the high morbidity of extended inguinal LN dissection (LND) or over-treatment of many patients. Therefore, inguinal dynamic sentinel node biopsy (DSNB) or modified LND is recommended by the European Association of Urology (EAU) guidelines to evaluate the nodal status of patients with clinically node-negative penile cancer. This study analyzed the reliability and morbidity of radioguided DSNB in penile can… Show more

“…Schubert et al. reported no patient having a false‐negative DSNB, albeit using a small cohort. As part of European Association of Urology and National Comprehensive Cancer Network guidelines, DSNB is part of the recommendation for non‐palpable inguinal lymph nodes in intermediate‐ and high‐risk disease .…”

“…Because of high morbidity from extended inguinal LND or over-treatment of many patients, inguinal-modified LND or sLND is recommended by the EAU guidelines to evaluate the nodal status of cN0 penile cancer patients. Excellent results concerning sensitivity, false-negative rates, and complication rates have been reported for radio-guided sLND in experienced hands [9][10][11]. However, dependence on radioisotopes limits this procedure to small parts of the developed world, and imposes restrictions on patient planning and hospital logistics.…”

“…Considering these recommendations radio-guided dynamic sentinel LN biopsy is a suitable procedure for LN staging in penile cancer. Patients can be spared from higher morbidity without compromising the detection of LN metastases or therapeutic implications ( 51 ). However, the procedure is still tedious and time-consuming requiring a dedicated nuclear medicine department and limited to certain center specialized in penile cancer treatment.…”
