Objective: To evaluate the clinical-radiographic subacromial disease symptoms correlated with adaptation of the measure of lateral acromion angle and its respective measuring radiographic acromial index. Methods: In the period between october 2010 and february 2011 were evaluated 55 painful shoulders with Neer test and true anteroposterior radiography. Patients were divided into two groups, with Neer test positive and negative. The index measuring the acromion and the lateral acromion angle have been standardized, and compared using statistical averages of 0.7 and 75°, respectively. Results: The predominant symptom in the population, females (72.73%), age less than 59 years (62.5%) and dominant side (65.31%). The acromion index above 0.7 was found to be symptomatic in 66.67% and lateral acromion angle less than 75 ° in 82.61%. When associated methods, 62.5% had positive clinical (p<0.083). Conclusion: The determination of radiographic acromial index and the lateral acromion angle together seem to be statistically associated with the disease of subacromial impingement.

The subacromial space is the anatomical region limited superiorly by the acromion and the coracoacromial ligament and inferiorly by the glenoid fossa and the humeral head. In humans, the main tissues that form it and that can be affected by subacromial impingement syndrome are the subacromial bursa and the supraspinatus tendon. The syndrome is the most common degenerative pathology affecting the human shoulder, and it is characterized by an uncertain etiopathogenesis. We compared different anatomical parameters of the scapula related to the subacromial impingement syndrome in humans, between modern humans and 2 species of African apes (Pan troglodytes and Gorilla gorilla). The 3 species we studied are orthograde primates and they present a similar structural pattern of the scapula and subacromial space, but anatomical differences exist owing to different types of locomotion used. The main differences indicate that African apes present a more curved and tilted acromion than humans, which does not, however, imply a difference in the relative size of the subacromial space among the 3 species studied. Humans also have a lower value of the relative size of the supraspinatus fossa than African great apes, and in human females have a relative lower value than males. We studied the anatomical structures of the shoulder in African apes that researchers have related to subacromial syndrome in humans to achieve a better understanding of the etiopathogenesis of the syndrome.

Background. The angle between the undersurface of the acromion and the glenoid face is defined as the lateral acromion angle (LAA) in a specific MRI sequence just posterior to the AC joint. Purpose. The purpose of this study was to examine reproducibility of the LAA in a conventional true ap radiograph. Patients and methods. The LAA was digitally measured on MRI and standard true ap radiographs for 64 consecutive patients (average age: 52.1 years; range 19-77 years) by two examiners. Results. A high correlation was shown between MRI (79.6° ± 5.5°) and conventional radiographs (80.1° ± 4.8°) with a Pearson Correlation Coefficent of 0.796 (p < 0.001). Interobserver reliability was good for MRI (ICC: 0.796; 95 % CI: 0.644-0.883) and excellent for Xray (ICC: 0.822; 95 % CI: 0.689-0.898). Overall we found that the LAA can be reliably reproduced in conventional radiographs in comparison with MRI. Conclusion. There was also good interobserver reliability for both MRI and conventional radiographs. Whether this can be a useful parameter in clinical practice has to be examined in future studies.
