Reliability of pressure pain threshold to discriminate individuals with neck and low back pain

Abstract: BACKGROUND: Pressure pain threshold (PPT) is decreased in several musculoskeletal disorders, giving indirect evidence regarding pain status. Despite the fact that PPT has been already proven to be reliable in patients with acute conditions, there is great variability of methods and results observed within studies, and only a few evidences confirming its reliability in chronic conditions. OBJECTIVE: The objective of this study was to determine the test-retest reliability of PPT in the neck and low back regions … Show more

“…Sensitivity to pressure (PPT) yields indirect but objective evidence for the experience of pain [ 62 ] and will be assessed with a digital pressure algometer (Wagner, Greenwich CT, USA; diameter 1 cm). The algometer will be applied 4 cm alongside the vertebral column, on a level to the process of vertebra L3, above the quadratus lumborum muscle.…”

“…Although the between-session design is recommended to reduce recall bias, [49][50][51] it cannot be used to determine the change needed to exceed the natural variability of PPT or TSP while testing pain modulation paradigms (eg, CPM and EIH since they are tested using a within-session design). In CLBP, tests and retests have been measured 24 hours 23 or 48 hours 24 apart. Similar to our results, ICC values of back sites reported 23,24 were excellent (ICC = 0.93 to 0.99).…”

“…Only 3 studies [22][23][24] tested the absolute reliability metrics for PPT (SEMeas and/or MDC ind ) in CLBP. However, comparisons with our results are limited because they used a different formula: (SEMeas = SD ICC 1 −…”

“…Intervertebral disc degeneration (IDD) can cause pathological changes, such as spinal stenosis, spinal segmental instability, osteophytes, and nerve root compression, which are the main causes of cervical and low back pain (LBP) [ 1 ], which, in turn, reduces the quality of life and work efficiency of patients and brings huge economic costs to the society [ 2 , 3 ]. IDD is associated with inflammatory mediators, abnormal metabolism of extracellular matrix (ECM), epigenetic changes of normal intervertebral discs (IVDs), decrease in the active cells, and increase in the senescent cells [ 4 , 5 ]; however, its pathological mechanism remains unclear.…”
