PurposeAn inherent problem on risk priority number (RPN) value duplication of traditional failure modes and effect analysis (FMEA) also exists in two customer-oriented FMEAs. One has no unique value, and another has 1% unique values out of 4,000 possible values. The RPN value duplication has motivated the development of a new customer-oriented FMEA presented in this paper to achieve practically all 4,000 unique values and delivering reliable prioritization.Design/methodology/approachThe drastic improvement is the result of power-law and VlseKriterijumska Optimizacija I Kompromisno Resenje (VIKOR). By having all three risk factors in a power-law form, all unique values can be obtained, and by applying VIKOR to these power-law terms, the prioritization is more practical and reliable.FindingsThe proposed VIKOR power law-based customer-oriented FMEA can achieve practically all 4,000 unique values and is tested with two case studies. The results are more logical than the results from the other two customer-oriented FMEAs.Research limitations/implicationsThe evaluation has been done on two case studies for the service sector. Therefore, additional case studies in other industrial sectors will be required to confirm the effectiveness of this new customer-oriented RPN calculation.Originality/valueAchieving all 1,000 unique values could only be done by having experts tabulate all possible combinations for the traditional FMEA. Therefore, achieving all 4,000 unique values will be much more challenging. A customer-oriented FMEA has been developed to achieve practically all 4,000 unique risk priority numbers, and that the prioritization is more practical and reliable. Furthermore, it has a connection to the traditional FMEA, which helps explain the traditional one from a broader perspective.

This paper introduces a formulation of appropriate risk estimation methods that can be used for improving of processes in the electronics area. Two risk assessment methods have been chosen with regard to the specifics of adhesive joining based on electrically conductive adhesives. The paper provides a combination of a failure mode and effect analysis (FMEA) and fault tree analysis (FTA) for optimizing of the joining process. Typical features and failures of the process are identified. Critical operations are found and actions for avoiding failures in these actions are proposed. A fault treehas been applied to the process in order to get more precise information about the steps and operations in the process, and the relations between these operations. The fault tree identifies potential failures of the process. Then the effects of the failures have been estimated by the failure mode and effect analysis method. All major differences between failure mode and effect analysis and fault tree analysis are defined and there is a discussion about how to use the two techniquescomplement each other and achieve more efficient results.

This work deals with the development and reduction of a fault tree applied to gearboxes of heavy commercial vehicles. The improvement with respect to the classical failure tree analysis (FTA) is to reduce the number of FTA components based on the sensitivity of the system reliability to the statistical parameters of the components failure models during a certain lifetime. These parameters determine the shape and slope of the decreasing reliability curve. The lifetime plays an important role because it is the base for any decision about guarantee time and periodic inspection in vehicle maintenance. As the components are usually in series configuration in this kind of mechanical system, the proposal is to consider only the components to which the system presents a significant sensitivity. A factorial planning with two replicates is applied to identify the system sensitivity with respect to these parameters taking into account the confidence interval in each case, as the parameters are evaluated from a sample with a specific size, which has a significant influence on the confidence limits. Therefore, the lifetime is defined in terms of kilometers and it must be economically viable for the company with respect to guarantee period, vehicle stops and maintenance costs. Following this objective, it is important to highlight that the reliability function sensitivity of the system changes depending on the lifetime and the behavior of each FTA component's reliability (failure model parameters). Then, the fault tree is assembled and successively reduced considering the sensitivity to scale ( ) and shape ( ) factors for components with Weibull distribution, as well as mean ( ) and standard deviation ( ) for Gaussian and Lognormal distributions, in specific kilometer intervals. This methodology allows a reliability model conception for management of the actions focused on products' guarantee and provides design descriptions for the development areas and manufacturing. In this model, it ispossible to obtain information about lifetime to assist in activities of performance studies and optimization in design engineering as well as the identification of problems related to design and manufacturing for several operation intervals. In the present case study, the methodology led to a 30% reduction on the number of components considered in the complete fault tree, contributing to guide future improvements to gearboxes reliability.
