Abstract.[Purpose] The purpose of this study was to clarify the factors affecting the burden of family caregivers of home-bound disabled elderly persons.[Subjects] Data were collected for 66 home-bound disabled elderly persons (mean age of 81.4 ± 9.7) and their 66 family caregivers (mean age of 62.4 ± 11. 5) who were living at home in Kobe city, Hyogo.[Method] Capacity to perform ADL was assessed using the Barthel Index for the home-bound disabled elderly persons. The care burden and health status of their family caregivers were also evaluated. [Results] There was no significant correlation between ADL, the grade of care required for home-bound disabled elderly persons and family caregivers' burden. It was also revealed that the family caregivers' burdens were determined by their state of depression.[Conclusion] These results suggest that social and psychological supports are necessary to reduce the burden of family caregivers.

BACKGROUND: There have been no studies of the relationship between violent behavior by older patients and the physical and mental health of caregivers. OBJECTIVE:To evaluate the influence of violent behavior in vulnerable elderly patients on bodily pain and caregiver burden in their informal caregivers. DESIGN:Cross-sectional study. SUBJECTS:One hundred thirty-seven patients aged ≥40 years old with limited activity and mobility in ten facilities providing home-care services in Japan.MEASUREMENTS: Degree of caregiver-perceived violent patient behavior and caregivers' bodily pain derived from a self-administered questionnaire, and caregiver burden assessed using scores from both the Burden Index of Caregivers and the Zarit Burden Interview. RESULTS:The mean age of the 137 patients enrolled in this study was 80.9 years. Of these patients, 31.4% were men, and 34.3% had violent behavior. The mean caregiver age was 65.0 years, and 29.2% were men. Caregivers who looked after violent patients experienced significantly higher odds of having bodily pain [AOR=3.51; 95% confidence interval (CI): 1.81 to 6.85]. Caregivers of violent patients also reported significant caregiver burden as assessed by the Burden Index of Caregivers (β-coefficient = 4.92; 95% CI: 1.95 to 7.88) and the Zarit Burden Interview (β-coefficient = 5.81; 95% CI: 2.92 to 8.70).CONCLUSIONS: Violent behavior among older patients is associated with significant increases in both physical and psychological burden in their informal caregivers.KEY WORDS: violent behavior; bodily pain; caregiver burden; informal caregivers.

Introducción. A pesar de la alta probabilidad de que los padres o cuidadores de niños asmáticos presenten sobrecarga, no se han validado instrumentos para medirla en el contexto del asma pediátrica. Objetivo. Iniciar la validación de una escala para medir la sobrecarga de los padres o cuidadores de niños asmáticos y determinar los factores asociados con esta sobrecarga. Materiales y métodos. Mediante un estudio analítico de corte transversal se midió el nivel de sobrecarga de los padres o cuidadores de niños asmáticos de 1 a 16 años, utilizando una versión abreviada de la escala de Zarit. Se hizo un análisis factorial exploratorio de los componentes principales de esta escala, y se evaluó su validez de constructo y su validez interna. Se ajustaron modelos de regresión logística para identificar los factores asociados con una sobrecarga intensa en los padres o cuidadores de los niños asmáticos. Resultados. La mediana (rango intercuartílico) de la edad de los 162 niños analizados fue de 6 años (rango, 4 a 9). Del total de padres o cuidadores, 37 (22,8 %) estaban experimentando sobrecarga intensa. El análisis factorial mostró una estructura probable de dos factores que, en conjunto, explican el 61,5 % de la varianza total de los resultados. La única variable que se asoció de forma independiente con una sobrecarga intensa en el análisis multivariado, fue el asma no controlada (OR=5,38; IC 95 % : 1,43-20,16; p=0,012). Conclusiones. La escala de Zarit utilizada tiene una aceptable validez interna y una adecuada validez de constructo para determinar el nivel de sobrecarga que experimentan los padres o cuidadores de niños asmáticos.
