BackgroundMost research on the health impacts of intimate partner violence (IPV) and child abuse has been conducted in Western countries and may not be generalizable to women living in different contexts, such as Saudi Arabia. Chronic pain, a disabling health issue associated with experiences of both child abuse and IPV among women, negatively impacts women’s well-being, quality of life, and level of functioning. Yet, the psychosocial mechanisms that explain how abuse relates to chronic pain are poorly understood. We developed and tested a theoretical model that explains how both IPV and child abuse are related to chronic pain.MethodsWe recruited a convenience sample of 299 Saudi women, who had experienced IPV in the past 12 months, from nine primary health care centers in Saudi Arabia between June and August 2015. Women completed a structured interview comprised of self-report measures of IPV, child abuse, PTSD, depressive symptoms, chronic pain, and social support. Using Structural equation modeling (SEM), we analyzed the proposed model twice with different mental health indicators as mediators: PTSD symptoms (Model 1) and depressive symptoms (Model 2).ResultsBoth models were found to fit the data, accounting for 31.6% (Model 1) and 32.4% (Model 2) of the variance in chronic pain severity. In both models, mental health problems (PTSD and depressive symptoms) fully mediated the relationship between severity of IPV and child abuse and chronic pain severity. Perceived family support partially mediated the relationship between abuse severity and depressive symptoms.ConclusionsThese results underscore the significance of considering lifetime abuse, women’s mental health (depressive and PTSD symptoms) and their social resources in chronic pain management and treatment.

The options of traditional self-report rating-scale, like the PTSD Checklist Civilian (PCL-C) scale, have no clear boundaries which might cause considerable biases and low effectiveness. This research aimed to explore the feasibility of using fuzzy set in the data processing to promote the screening effectiveness of PCL-C in real-life practical settings. The sensitivity, specificity, Youden’s index etc., of PCL-C at different cutoff lines (38, 44 and 50 respectively) were analyzed and compared with those of fuzzy set approach processing. In practice, no matter the cutoff line of the PCL-C was set at 50, 44 or 38, the PCL-C showed good specificity, but failed to exhibit good sensitivity and screening effectiveness. The highest sensitivity was at 65.22%, with Youden’s index being 0.64. After fuzzy processing, the fuzzy-PCL-C’s sensitivity increased to 91.30%, Youden’s index rose to 0.91, having seen marked augmentation. In conclusion, this study indicates that fuzzy set can be used in the data processing of psychiatric scales which have no clear definition standard of the options to improve the effectiveness of the scales.

The aim of this study was to investigate the influence of a polymorphism in the serotonin transporter gene (5-HTTLPR) in patients diagnosed with posttraumatic stress disorder (PTSD) in a Chinese sample of earthquake survivors. Polymerase chain reaction (PCR) amplification and amplified fragment length polymorphism (AFLP) were performed to type 5-HTTLPR promoter polymorphism in 57 PTSD patients and an equal number of healthy controls. The genotype and allele frequency distribution were analyzed and compared using various statistical methods. The frequency of LL, SL and SS genotypes in patients was found to be 5, 16 and 36 respectively, in comparison to 16, 22 and 19 in healthy controls. Fewer patients tended to be L genotype (22.8%) than controls (47.4%), but the number of patients with the S genotype was higher (77.2%) compared to controls (52.6%). The results show a statistically significant difference in genotype and allele frequency distribution between patients and controls. This research suggests that PTSD symptoms are significantly associated with 5-HTTLPR genetic polymorphism. These results add to the important research of genetics of psychiatric disorders, particularly in a Chinese context that has not been previously studied.
