Systemic inflammation and immune response were associated with prognosis of tumors. However, data was limited due to the relatively low incidence of gastrointestinal stromal tumors (GISTs). The aim of the present study was to investigate the predictive value of preoperative peripheral blood cells in prognosis of GISTs.From September 2008 to July 2015, a total of 274 GIST patients in our department were enrolled in the present study. Clinicopathological features of GISTs were recorded. The association between preoperative peripheral blood cells and prognosis of GISTs were analyzed.Tumor location, tumor size, mitotic index, intratumoral necrosis, and National Institutes of Health (NIH) risk category were associated with prognosis of GISTs. High neutrophil-to-lymphocyte ratio (NLR), monocyte-to-lymphocyte ratio (MLR), platelet-to-lymphocyte ratio (PLR), neutrophil-to-white blood cell ratio (NWR), monocyte-to-white blood cell ratio (MWR) and low lymphocyte-to-white blood cell ratio (LWR) was associated with poor prognosis of GISTs (76.2% vs 83.7%, P = 0.010. 70.5% vs 98.7%, P = 0.000. 65.7% vs 96.4%, P = 0.004. 78.5% vs 82.5%, P = 0.044. 73.5% vs 97.8%, P = 0.004. 76.6% vs 83.6%, P = 0.012, respectively). However, tumor size was the only independent risk factor for prognosis according to multivariate analysis (P = 0.006). Tumor location, tumor size, mitotic index, and NIH risk category were significantly correlated with the above-mentioned parameters (all P < 0.05). The prognosis of GISTs with tumor size >5 cm, high MLR, high PLR, and high MWR was significantly lower than the remnant patients (P = 0.010).The peripheral blood routine test is convenient, reproducible, and inexpensive. High NLR, MLR, PLR, NWR, MWR, and low LWR were associated with poor prognosis of GISTs. The association between the above parameters and prognosis of GISTs may be attributed to their correlation with tumor size, mitotic index, and NIH risk category. The combination of tumor size, MLR, PLR, and MWR could further increase the predictive value of prognosis of GISTs.

ÖZET Amaç: Granülomatöz mastit, hem tanısı zor hem de ideal tedavi yöntemi olmayan kronik inflamatuar bir hastalıktır. Nadir görülen bir antite olması nedeni ile prognostik faktörler ve tedavi kalsifikasyonu üzerine araştırmalar kısıtlıdır. Son zamanlarda sistemik inflamasyonun biyobelirteci olarak kullanılan nötrofil-lenfosit oranı (NLO) nın popülerliği giderek artmaktadır. Bu çalışmada, kronik inflamatuar hastalık olması nedeni ile granülomatöz mastitte tedavi sonrası nükslerin öngörülmesinde tedavi öncesi NLO'nun rolünün araştırılması amaçlanmıştır. Gereç ve Yöntemler: Biyopsi sonuçlarına göre granülomatöz mastit hastalığı tespit edilen ve tedavi edilen 62 hastanın verileri retrospektif olarak incelendi. Hastalar, medikal tedavi ve cerrahi tedavi olmak üzere 2 gruba ayrıldı. Demografik verilerinin yanında tedavi öncesi NLO'ları hesaplandı. Her iki grupta da tam kür sağlanmış olan hastalar çalışmaya dâhil edildi. Tedavilerin tamamlanmasından en az 6 ay sonra hastalar telefon yoluyla aranarak nüks durumları, tekrar tedavi alıp almadıkları sorgulandı. Bulgular: Toplam 59 hasta çalışmaya dâhil edildi. Hastalar tedavi sonrası en az 6 ay takip edilmiş olup, ortalama takip süresi 52,44±27,29 (6-103) ay idi. Steroid tedavisi alan hastaların 4 (%20)'ünde nüks gelişirken, cerrahi tedavi alan hastaların 3 (%7,9)'ünde nüks gözlendi. Cerrahi ve medikal takip edilen hasta grupları arasında, tedavi öncesi NLO ile nüks açısından anlamlı farklılık izlenmedi (p>0,05). Ancak yapılan tedaviye bakılmaksızın incelendiğinde, nüks olan hastaların NLO'ları nüks olmayanlara göre daha yüksekti (p=0,00). Sonuç: Granülomatöz mastit hastalarında tedavi öncesi NLO değeri uzun dönemde nüksü öngörmede fikir verici olabilir. Background and Objectives: Several inflammation markers were found to have a prognostic value in cancer. We investigated the significance of preoperative white cell ratios in determining gastrointestinal stromal tumors (GISTs) outcome. Methods: Clinicopathological features of patients who underwent surgery for GIST were reviewed. The following peripheral blood inflammation markers were calculated: neutrophil-lymphocyte ratio (NLR), monocyte-lymphocyte ratio (MLR), platelet-lymphocyte ratio (PLR), neutrophil-white blood cell ratio (NWR), lymphocytewhite cell ratio (LWR), monocyte-white cell ratio (MWR), and platelet-white cell ratio (PWR). Results: We analyzed 127 patients. Three-and five-year disease-free survival (DFS) were 89.7% and 86.9%, respectively. The univariate analysis selected tumor diameter (P = 0.003), gastric location (P = 0.024), cell type (P = 0.024), mitosis (P < 0.001), MLR (P = 0.014), NLR (P = 0.016), and PLR (P = 0.001) as the factors associated to DFS. The independent prognostic factors for DFS were mitosis (P = 0.001), NLR (P = 0.015), MLR (P = 0.015), and PLR (P = 0.031), with MLR showing the highest statistical significance and hazard ratio (HR) value. MLR, NLR, and PLR were the only prognostic factors in the subgroup of patients with moderate to high Miettinenʼs risk class. A high value of MLR was associated with reduced DFS. Conclusion: MLR, NLR, and PLR are independent prognostic factors for DFS in GISTs.We first demonstrated the role of MLR as a predictor of recurrence in GIST. Its inclusion into clinical management may improve the recurrence estimation.
