This review argues that (1) the habitual mandibular position is constantly variable and so cannot be considered as a craniomandibular reference point, (2) there is no unique centric relation, (3) mandibular posture greatly depends on head posture, (4) clinical evaluation of the occlusal vertical dimension is mostly empirical, and (5) neither the vertical dimension at rest nor the centric relation can be determined by means of existing instrument-based clinical methods. However, some physiological conditions exist that facilitate the recording of craniomandibular position.

ABSTRACT:The purpose of this study is to assess the relation between the speaking space of the /s/ sound and the freeway space in two subject groups. One group had natural dentition (Group I, n = 61) and the other comprised complete denture wearers (Group II, n = 33). The analysis was done by means of a jaw-tracking device (K6-I Diagnostic System, Myotronics Research Inc., Seattle, WA, USA). Freeway space was determined by asking the subjects to occlude from the postural rest position. Speaking space of /s/ was measured during the pronunciation of the word "seis" and comprised the mean distance from the /s/ speaking position to maximal intercuspation. A weak correlation was found between the speaking space of /s/ and the freeway space in Group I (r = 0.41, p < 0.01), but in Group II, the correlation was stronger (r = 0.75, p < 0.01). The speaking space of /s/ and freeway space were different in Group I, but statistically similar in Group II (paired t-test, α = 0.05). It can be suggested that anatomic changes following prosthetic procedures caused a functional adaptation which resulted in more similar values for the speaking space of /s/ and the freeway space. RESUMO:O objetivo deste estudo foi investigar a associação entre o espaço de pronúncia do som /s/ e o espaço funcional livre em dois grupos de indivíduos. A amostra foi classificada em dentados (Grupo I, n = 61) e portadores de próteses totais (Grupo II, n = 33). A análise foi realizada empregando um instrumento cinesiográfico (K6-I Diagnostic System, Myotronics Research Inc., Seattle, WA, EUA), capaz de fornecer registros dos movimentos mandibulares. O espaço funcional livre foi determinado solicitando-se ao indivíduo que ocluísse a partir da posição de repouso postural. O espaço de pronúncia do /s/ foi aferido durante a pronúncia da palavra "seis", registrado como a posição mandibular média para o som /s/ em relação à máxima intercuspidação. Houve uma fraca correlação entre o espaço funcional livre e o espaço de pronúncia do /s/ para o Grupo I (r = 0,41, p < 0,01), mas uma correlação relativamente forte para o Grupo II (r = 0,75, p < 0,01). O espaço funcional livre e o espaço de pronúncia do /s/ foram diferentes no Grupo I, mas estatisticamente semelhantes no Grupo II (teste t pareado, α = 0,05). Dessa forma, pode-se deduzir que mudanças anatômicas após o procedimento protético provocaram uma adaptação funcional, sendo que esse processo adaptativo aproximou as duas variáveis -espaço de pronúncia do /s/ e espaço funcional livre.

resUMen objetivo: Validar en población chilena, el método antropométrico indio de Ladda que estableció en la población chilena una correlación positiva entre la dimensión vertical oclusal y longitud de ciertos dedos de la mano. Materiales y métodos: Estudio de tipo transversal, 151 adultos jóvenes, 76 hombres y 75 mujeres. Se replicó el trabajo referencial, utilizando un pie de metro digital, realizando mediciones de los dedos índice y meñique y la distancia desde la punta del pulgar a la punta del índice de la mano derecha, correlacionando las mediciones obtenidas con la dimensión vertical oclusal y aplicando el coeficiente de correlación de Pearson. resultados: La dimensión vertical oclusal presenta correlación estadísticamente significativa con los parámetros estudiados. En hombres, la correlación fue más consistente con la longitud del dedo índice (r=0.908), mientras que, en las mujeres, fue más consistente con la longitud del dedo meñique (r=0.827). conclusiones: Las variaciones obtenidas caen dentro del intervalo de 3-4mm, estando en consonancia con otros métodos antropométricos publicados, confirmando la predictibilidad de la dimensión vertical oclusal obtenida mediante este método objetivo. Materials and methods: A cross-sectional study, with 151 Chileans: 76 males and 75 females. Anthropometric measurements of vertical dimension of occlusion, length of index finger, length of little finger, and distance from tip of thumb to tip of the index finger of the right hand were recorded clinically using digital Vernier caliper. Correlation was studied using Pearson's coefficient. results: Vertical dimension of occlusion was significantly correlated with all the parameters analyzed. In males, correlation of vertical dimension of occlusion was stronger with the length of the index finger (r=0.908) whereas in females, it was stronger with the length of the little finger (r=0.827). conclusions: Since the variations between vertical dimension of occlusion and finger lengths are within the range of 3-4 mm, as with other anthropometric methods published, VDO prediction through this method is reliable, and reproducible.
