Relationship of Musculotendinous Junction Location to Harvested Semitendinosus and Gracilis Tendon Length

Abstract: Background: Harvested hamstring tendon length has received scant attention in published anterior cruciate ligament (ACL) reconstruction literature, yet length can limit the ability to increase graft diameter by folding the tendon over more than once. Indeed, some ultrashort tendons may be too short to yield a clinically useful graft after being folded over just once. Ultimately, the total length of a harvested hamstring tendon may depend on the length of the tendon distal to its musculotendinous (MT) junction.

“…They previously identified that a gross "tendon only length" >9.5 cm for the semitendinosus (ST) or 9 cm for gracilis (G) would be at very low risk of being too short in total tendon length for a robust ACL reconstruction. 6 The 95% confidence interval of the current study is quite large (3 cm) because of the high variability in the obtained lengths (9-17 and 9-15 cm for the ST and G, respectively). Therefore, the authors suggest a preoperative tendon-only length MRI measurement of >12.5 cm for the ST and 12 for the G to be confident that the total harvested tendon would not be <15 cm.…”

“…Furthermore, a minimum length of 240 mm of semitendinosus and 160 mm of the gracilis tendon is required to prepare the 5S configuration. 17 Ilahi et al 15 noted that abnormally distal musculotendinous junctions of hamstring tendons can cause shorter grafts in the absence of technical error and affect the ability to employ a particular graft preparation technique, thus increasing uncertainty of graft utilization. Our study showed a preponderance of female patients and those of shorter stature and with lower weight undergoing the 5S graft configuration.…”
