Toxicological examination in forensic practice is important for the proper diagnosis of acute poisoning. We have discussed the properties and features of poisoning incidents due to gaseous substances and elaborated on the simplified analytical techniques and apparatus used for their identification and quantitation for forensic purposes. Briefly, we have explained the simplified analysis of toxic gaseous substances such as carbon monoxide, hydrogen cyanide, hydrogen sulfide, and helium in blood. The techniques used include color testing, gas chromatography, detector tube, oximeter, and spectrophotometric method. In doing so, we have shared our experiences and highlighted the fact that the analysis of gaseous substances can be performed using readily available laboratory tools and equipment. We have emphasized the need and usefulness of the reference data tables for guiding forensic diagnosis. We hope that the above overview will assist other colleagues to implement such simplified techniques for the advancement of forensic medicine practice.

Ö ÖZ ZE ET T Oksijen satürasyonu günümüzde, özellikle kritik hastaların değerlendirilmesinde beşinci vital bulgu olarak kabul edilmektedir. İnsan gözü, hipoksiyi değerlendirme ve erken tanımada yetersizdir. Bu amaçla kullanılan nabız oksimetre cihazları 1900'lü yıllarda geliştirilmiş, günü-müzde güncelliğini koruyan, kritik hastada hastanın oksijen satürasyonu değerini hızlı, girişim gerektirmeden ve ağrısız bir şekilde belirlememizi sağlayan cihazlardır. Çocukluk yaş grubunda ağrılı işlemler, özellikle solunum sıkıntısı olan çocuklarda solunum çabasının daha da artmasına ve hipokseminin derinleşmesine neden olabilmektedir. Günümüzde, yenidoğanlarda kardiyak tarama amaçlı da kullanım alanı bulan nabız oksimetre cihazları kolay ulaşılabilir, ucuz, farklı boyutlarda ve taşınabilir özelliktedir. Aynı zamanda, kritik hasta takibinin yapıldığı acil servis gibi bölümlerde hastaların sürekli monitörizasyonunu sağlayabilmektedir. Çeşitli tipleri bulunan nabız oksimetre cihazları temelde aynı prensiple; oksijen taşıyan hemoglobin ile oksijen taşımayan hemoglobinin iki farklı dalga boyundaki ışık emiliminin ölçümüyle çalışmakta-dır. Ancak, probun yanlış yerleştirilmesi, hareket artefaktı ve ortam aydınlığı gibi cihaza ait nedenler ya da hemoglobinopatiler, cilt pigmentasyonu ya da tırnakta oje olması gibi hastaya ait nedenler ölçüm hatalarına neden olabilmektedir. Tüm sağlık çalışanlarının, nabız oksimetre cihazlarını etkin bir şekilde kullanabilmesi için, nabız oksimetre cihazlarının hangi amaçla, nasıl kullanıldığını ve kullanırken nelere dikkat etmesi gerektiğini bilmeleri gerekmektedir. The human eye is inadequate to assess and diagnose early hypoxia. Pulse oximetry devices were developed in 1900, still keeping actuality, permit to determine the oxygen saturation quickly without interference and pain in critically ill patients. Painful procedures, especially in children with respiratory distress, could lead to increase in respiratory effort and deepening in hypoxia. Recently, pulse oximetry devices are also used for cardiac screening in newborns. The devices are cheap, portable, readly available, and in different sizes. They also provide continuous monitoring of patients in departments where clinically ill patients are followed such as pediatric emergency department. Various types of pulse oximetry devices work in the same working principle; calculation of absorption of oxyhemoglobin and deoxyhemoglobin in different wavelength. RESUMENLa intoxicación por gases en nuestro medio es un problema importante debido a su alta incidencia. En el caso concreto de la intoxicación por monóxido de carbono, es la principal causa de muerte por intoxicación involuntaria en nuestro medio, muchas veces coexistiendo con una intoxicación por cianuro. Ambas intoxicaciones pueden ser de carácter grave, basándose su diagnóstico en la mera sospecha del médico. Además, su importancia radica en que ambas intoxicaciones tienen un tratamiento concreto. La oxigenoterapia normo o hiperbárica es el tratamiento de elección de la intoxicación por monóxido de carbono. En el caso de la intoxicación por cianuro, la hidroxocobalamina es hoy día el tratamiento de elección, ya que ha demostrado ser un eficaz antídoto.Palabras clave: Intoxicación. Monóxido de carbono. Cianuro. Oxigenoterapia. Hidroxocobalamina. ABSTRACTPoisoning by gases in our area is an important problem due to its high incidence. In the specific case of carbon monoxide poisoning, this is the main cause of death by poisoning in our environment, on many occasions coexisting with cyanide poisoning. Both poisonings can be severe, their diagnosis being based on the mere suspicions of the doctor. Besides, their importance lies in the fact that both poisonings have a very specific treatment. Normo or hyperbaric oxygenotherapy is the treatment for carbon monoxide poisoning. In the case of cyanide poisoning, hydroxocobalamin is nowadays the treatment of choice, since it has proved itself to be an efficient antidote.
