Relationship between risk assessment and payment models in Swedish Public Dental Service: a prospective study

Abstract: BackgroundTo a) compare risk categories in patients selecting a capitation payment (CP) model with those in fee-for-service (FFS), b) determine the 3-year caries increment in the two groups, and c) compare the amount of delivered preventive care in the two groups.MethodsA comprehensive risk assessment was carried out in 1295 young adults attending eight Public Dental Clinics in the Scania region and 75% could be re-examined after 3 years; 615 had selected the CP model and 310 the traditional FFS. Caries was sc… Show more

“…In a study on the oral ill health risk in FFS and DCH among young individuals in Sweden, Peterson & Twetman raised a discussion about the risk for divergent health development in the two payment models over time [ 23 ]. A free choice is likely to put the group of already resourceful individuals in a better position than those less resourceful to choose their most advantageous alternative.…”

“…54 In Sweden, some argue that the current capitation function or service-purchasing model may contribute to or increase inequality. 55 Health economics are increasingly interested to expand evaluation of cost-effectiveness in integrated care for chronic conditions. 56 In the UK, the Quality and Outcomes Framework (QOF) pay for performance (P4P) scheme was explored as a potential model to reward primary care practitioners.…”

“…In the ideal capitation model, several measures such as age, gender, morbidity, additional health needs, local labor costs, rurality, patient turnover, and so on can be included and comprehensively examined to predict patient expenditure and base capitation on the prediction 54 . In Sweden, some argue that the current capitation function or service‐purchasing model may contribute to or increase inequality 55 . Health economics are increasingly interested to expand evaluation of cost‐effectiveness in integrated care for chronic conditions 56 .…”

“…The price range for CCS enrolment is based on risk assessments, carried out by the dental health care provider at a baseline examination. Peterson & Twetman [ 14 ] found that patients assigned to a higher risk group chose not to enroll, while patients assessed as low risk did. However, various types of risk assessments programs are used within the PDHS regions [ 15 ].…”

“…Apart from the patients’ own choice, this has further exacerbated the pronounced variations in dental attendance nationwide [ 8 , 19 , 26 , 27 ]. It is of interest to note that dental attendance by CCS patients is higher than those in FFS [ 12 , 14 ]. In addition to this, the CCS patients are more satisfied with their payment plan than the FFS patients [ 16 , 21 , 23 ].…”
