Background: Keratoconus, a corneal ectasia, is associated with corneal thinning and altered optical media. Consequently, assessment of the visual field, optic nerve head and intraocular pressure measurements may be challenging in patients with keratoconus. Few studies have investigated posterior segment variables including the retinal nerve fibre layer (RNFL) and ganglion cell complex (GCC) thickness in patients with keratoconus.Aim: To investigate RNFL and GCC thickness in patients with keratoconus.Methods: A comparative quantitative research design was used. The sample consisted of 56 participants (28 with mild, moderate or severe keratoconus, and 28 controls) who accessed the optometry clinic at the University of KwaZulu-Natal. There was an equal distribution of male (n = 14) and female (n = 14) participants in the keratoconus and control groups. Most participants were black (n = 34) or Indian (n = 18). Corneal power and refractive error were assessed with the Oculus Keratograph and subjective refraction respectively. The iVue-100 optical coherence topography device was used to measure RNFL and GCC thickness. Data were analysed by descriptive and inferential statistics.Results: The mean global RNFL thickness was slightly higher in the control group than the keratoconus group for the right (106 µm vs. 99 µm) and left (103 µm vs. 98 µm) eyes but these differences were not significant (p ≥ 0.057). For all RNFL quadrants, slightly lower mean RNFL measurements were found in the keratoconus group. The mean GCC thicknesses were marginally higher (3 µm – 6 µm) in the control group.Conclusion: The RNFL and GCC thickness differences between patients with keratoconus and controls are not clinically significant. Therefore, abnormally reduced RNFL and GCC thickness measurements in patients with keratoconus warrant further investigation for other pathologies specifically glaucoma.

Purpose To review the ability of current imaging technologies to provide estimates of rates of structural change in glaucoma patients.Patients and methods Review of literature. Results Imaging technologies, such as confocal scanning laser ophthalmoscopy (CSLO), scanning laser polarimetry (SLP), and optical coherence tomography (OCT), provide quantifiable and reproducible measurements of the optic disc and parapapillary retinal nerve fibre layer (RNFL). Rates of change as quantified by the rim area (RA) (for CSLO) and RNFL thickness (for SLP and OCT) are related to glaucoma progression as detected by conventional methods (eg, visual fields and optic disc photography). Evidence shows that rates of RNFL and RA loss are significantly faster in progressing compared with nonprogressing glaucoma patients. Conclusion Measurements of rates of optic disc and RNFL change are becoming increasingly precise and individualized. Currently available imaging technologies have the ability to detect and quantify progression in glaucoma, and their measurements may be suitable end points in glaucoma clinical trials.

RESUMENObjetivos: Analizar cambios en el espesor de la capa de fibras nerviosas de la retina (CFNR) peripapilar tras cirugía de cataratas. Métodos: Medición del espesor medio de la CFNR de pacientes sometidos a cirugía de cataratas utilizando tomografía de coherencia óptica (OCT) III Stratus ® antes de la cirugía, un día después de la intervención y transcurrido un mes. Los datos fueron analizados mediante el programa informático SPSS 12.0. Los resultados fueron comparados empleando el test pareado de dos colas t de Student. Resultados: El grupo de estudio estaba compuesto de 74 ojos de 74 pacientes. El espesor medio de la CFNR antes de la cirugía de cataratas (AVGT0) fue 90,71 µm (DE: 19,93), el día después de la cirugía (AVGT1) fue 88,30 µm (DE: 20,59); y un mes después (AVGT2) 97,45 µm (DE: 14,30). No se encontraron diferencias estadísticamente significativas entre AVGT0 y AVGT1 (P=0,37); sin embargo, dicha diferencia sí se encontró entre AVGT1 y AVGT2 (P= 0,002).
