Relationship between maternal hemodynamics and plasma natriuretic peptide concentrations during pregnancy complicated by preeclampsia and fetal growth restriction

Abstract: The elevated NT-proBNP in preeclampsia may reflect ventricular stress and subclinical cardiac dysfunction worsening if FGR is present. This may have implications for the acute management of the preeclampsia and FGR women and for appropriately timed therapeutic interventions later in life.

“…Cardiac assessment may also prove useful for guiding antihypertensive therapy and improving outcomes for women with preeclampsia. 7982 The choice of antihypertensive agent varies between national guidelines despite the fact that drugs of choice have vastly different mechanisms of action and side effect profiles. For instance, labetalol is the first line drug for treatment of pregnancy hypertension in United Kingdom.…”

“…NT-proBNP levels are associated with the phase of left ventricular dysfunction; therefore, they reflect the degree of heart failure [19]. It has been determined that NT-proBNP levels are high in women with late-onset preeclampsia [20]. It has been reported that maternal hemodynamic status worsens and NT-proBNP levels are even higher in pregnant women with preeclampsia accompanied by IUGR [21].…”

“…Recently a new device has been introduced for the measurement of SV and CO in pregnancy, the UltraSonic Cardiac Output Monitor (USCOM 1A®). This is a non‐invasive hemodynamic monitor that measures cardiovascular values by using a non‐imaging continuous‐wave Doppler transducer placed on the suprasternal notch to determine ascending transaortic blood flow (Figure ).…”
