OBJECTIVE: To establish the relationship between biochemical, serological and viral replication indices in an individual with incidentally detected asymptomatic HBsAg positive. METHOD: This cross-sectional study was undertaken on 73 asymptomatic HBsAg positive individuals between January 2010 and December 2011. They were divided into 5 groups on the basis of HBeAg status, ALT levels, and HBV DNA levels. RESULTS: There was statistically significant difference in HBV DNA levels and ALT between HBeAg positive and negative; and in ALT levels between cases in Group C and E and that between Groups C & D. There were significant difference in DNA levels between group D & group E and that between group C & group E. HBV DNA levels Corelated significantly with grade (r = 0.48, p <0.001) with Stage (r = 0.47, p <0.001) with ALT (r = 0.37, p <0.001) and AST (r = 0.42, p <0.001). AST co-related significantly HBV DNA levels. There was NO statistically significant difference in HBV DNA levels in cases with elevated ALT(> 40 IU/ml) and those with normal ALT(<40 IU/ml). CONCLUSION: ALT and AST had a strong correlation with HBV DNA level. INTRODUCTION:Hepatitis B Virus (HBV) infection and its sequel are serious public health problems worldwide. Around 2 billion people are infected by hepatitis B virus (HBV) worldwide of which 400 million persons suffer chronic infection with HBV. 1 HBV infection is usually clinically in apparent. Approximately, 5-10% of infected adults develop chronic liver disease of varying severity. More than 90% of infants infected during the first year of life and 30-50% children infected between 1-4 yrs of age develop chronic infection. The risk of death from HBV related liver cancer and cirrhosis is approximately 25% in persons who become chronically infected during child hood. 2, 3 15-40% of carriers will develop serious complications during lifetime like cirrhosis, decompensated liver disease or HCC. 1 75% of carriers are present in Asia. India has the second largest pool of HBsAg carriers after China.

Background: Chronic infection with hepatitis B virus causes a spectrum of diseases ranging from asymptomatic infective state to cirrhosis and hepatocellular carcinoma. The detection and quantification of HBV DNA plays an important role in diagnosing HBV infection as well as monitoring therapeutic responses. Methods: The aim of this study was to detect and quantited HBV DNA by real time PCR and to find out correlation HBeAg with HBV DNA in chronic carriers: The present study carried out among 61 previously diagnosed chronic hepatitis B patients. Results: The mean age of study population was 29.20 years. Among the 61 cases, 13(21.3%) were HBeAg positive and 48(78.7%) were HBeAg negative. By real time PCR, DNA detected in 45(73.8%) patients and 16(26.2%) patients were undetected. Association of HBeAg and HBV DNA was observed in 13 HBeAg positive cases, where 12(92.3%) had detectable DNA and in 48 HBeAg negative patients, 33(68.8%) had detectable DNA while 15(31.2%) were undetected. The present study observed a higher viral load (10 5 copies/ml) among HBeAg positive patients (84.6%) than HBeAg negative patients (12.5%). Conclusion: The present study observed that, there was positive correlation among HBeAg and HBV DNA in chronic HBV carriers. However, there was some discordance observed among HBeAg and HBV DNA. Therefore, in addition to HBeAg, HBV DNA should be assessed for appropriate evaluation of CHB carriers.

Introduction: The outcome of chronic hepatitis B (CHB) infection in a cohort of CHB patients at the hepatology clinics of King Abdul Aziz University Hospital in Jeddah was studied. Methodology: The results of a complete blood count, prothrombin time, liver function test and hepatitis B virus polymerase chain reaction (HBV-PCR) performed over the previous two to five years of follow-up were reviewed. Results of abdominal ultrasound performed within the last year and the data on the treatment type, duration and resistance were also obtained. Results: The majority of the 109 patients studied were Hepatitis B e antigen (HBeAg-negative; 87.2%). Male patients had higher serum ALT values compared to females at follow-up. HBeAg-positive patients had higher HBV-PCR levels at diagnosis compared to HBeAgnegative patients. Patients below 40 years of age had higher HBV-PCR compared to those above 40 years. Ultrasound showed liver cirrhosis in 11% of patients. Cirrhotic patients had higher GGT levels compared to non cirrhotic patients. Drug resistance developed in 25% of the 20 Lamivudine-treated patients. The mean duration of treatment was 2.5 ± .47 years. Conclusion: CHB was predominantly HBeAg negative, with a benign long-term outcome in most patients. Therapy may need to be individualized for patients with high risk of progression.
