Relationship between Heberden's nodes and underlying radiographic changes of osteoarthritis

Abstract: Objective: To determine whether clinically determined Heberden's nodes (HN) and Bouchard's nodes (BN) are associated with underlying individual radiographic changes of osteoarthritis (OA). Methods: 232 index patients with symptomatic large joint and/or hand OA, and 257 of their first degree relatives were included. HN were graded 0-2; BN were scored as present/ absent. Joint space narrowing (JSN) and osteophyte (OST) were each scored 0-3 using the OARSI atlas. A weighted k test was used to examine intraobserve… Show more

“…Heberden nodes (HN) and Bouchard nodes (BN) associate with underlying structural changes of HOA, especially osteophyte (OR = 5.15, 95% CI 4.37 to 6.08) 5860. However, their sensitivity and specificity for HOA vary widely from 0.3 to 0.9 depending on the cut-off grade, gold standard and control subjects.…”

“…The nodes affecting the thumb IP and metacarpophalangeal (MCP) joints were also documented. This method has been previously validated (7,13). Intraobserver reproducibility was determined by the metrologists' reassessment every 2-3 months.…”

“…Multiple nodes are regarded widely as a marker for generalized OA (4). Few studies have investigated the association between hand nodes and underlying structure changes (5)(6)(7)(8). In the studies that have investigated this, the measures used to assess the association were inconsistent, which led to a different clinical interpretation of the association.…”
