Relationship between elevated immunoglobulin free light chain and the presence of IgH translocations in multiple myeloma

Abstract: Elevated immunoglobulin free light chain (FLC) level and abnormal FLC ratio is commonly seen in multiple myeloma (MM) and have prognostic implications. We hypothesized that presence of IgH translocations leads to unbalanced production of light chains and more extreme abnormalities of FLC and may explain the prognostic value of FLC. We studied 314 patients with newly diagnosed MM enrolled on a phase-III trial, in whom results of FISH testing and serum FLC were available. Cytogenetic analyses and FLC estimates w… Show more

“…27 However, they also found that the negative prognostic effect of very abnormal FLC levels was independent of the translocation abnormality [t(4;14) and t(14;16)]. 27 In our data set, IgH translocations were only available in 277 patients (53.1% of relapsed population) and, therefore, fluorescence in situ hybridization data were not included in the multivariate analysis.…”

“…[2][3][4] One study showed that cases with IgH translocations had higher FLC levels and abnormal FLC ratio. 5 Recently, prognostic significance was shown for isotype matched Ig ratios (e.g. IgGκ/IgGλ).…”
