Relationship between Cycloid Psychosis and Typical Affective Psychosis

Abstract: This article describes the clinical features and outcome of a series of 73 inpatients who fulfilled Perris’ criteria for cycloid psychosis. It is argued that although the cases differ from either schizophrenia or affective psychosis in some ways, the condition of cycloid psychosis is best regarded as an atypical variety of affective psychosis.

“…A comparable overlap with mood disorders was reported in a reanalysis of the original Perris patient population [30]. During the acute phase of CP, an overlap of 20–25% with schizophrenia was demonstrated [5, 6]. The latter may be explained by the absence of a criterion of the duration for CP.…”

“…The nosological separation from other endogenous psychoses and its prognostic validity were stressed [3]. These observations were confirmed in subsequent studies, although an overlap with affective disorders was found [4, 5, 6]. …”

“…More specifically, its nosological status within the broad field of psychotic disorders, and particularly in relation to mood disorders and schizophrenia, continues to be a matter of debate. Studies addressing this question have supported the view of cycloid psychosis as an independent nosological entity [1,3,20,31], as an atypical variant of affective psychosis [32], as an atypical form of schizophrenia [33], and as a heterogeneous condition [14,16,17]. Finally, other studies have yielded inconclusive results [34,35].…”

“…Da ihre Abgrenzbarkeit gegenüber schizophrenen Erkrankungen inzwischen durch eine Reihe von Befunden gut bestätigt werden konnte [2,9,10,31,32], rückt nun ihre Unterscheidbarkeit gegenüber bipolaren affektiven Psychosen in den Blickpunkt des Interesses, zumal zykloide Psychosen von verschiedenen Autoren als atypische Varianten bipolarer affektiver Erkrankungen angesehen werden [6,7,15,16,25].…”
