Relationship between cold pressor pain-sensitivity and sleep quality in opioid-dependent males on methadone treatment

Abstract: Aim. Poor sleep quality due to pain has been reported among opioid-dependent male patients on methadone maintenance therapy (MMT) but objective pain data are lacking. This study aimed to investigate the rate of pain-sensitivity using cold pressor test (CPT) and the relationship between pain-sensitivity and sleep quality in this population. Methods. A total of 168 male participants were included into the study. Objective pain-tolerance was evaluated at 0 h and at 24 h after the first CPT. Malay version of the P… Show more

“…Interestingly, one study reported that the majority of opioid-dependent male patients on methadone treatment were found to be “pain-sensitive” with the CPT [33]. Poor sleep quality has been associated with cold pressor pain-sensitivity as well [33].…”

“…South‐East Asia is a highly populated and a culturally diverse region, with ethnic Malays comprising the largest population group, mainly populating countries including Malaysia, Indonesia, and southern part of the Philippines. We have previously revealed hyperalgesia among opioid‐dependent Malay males on methadone therapy and that the pain is associated with poor sleep quality . Polymorphisms in the OPRM1 and CYP2B6 genes were found to be associated with cold pain responses in this population .…”

“…The CPT method utilized in the current study was adapted from Chen et al and Compton et al and had been described elsewhere . Briefly, a 48‐quart cool box was filled with a mixture of two‐thirds crushed ice and one‐third tap water.…”

“…Sleep quality has also been found to be poorer among opioid‐dependent individuals on methadone maintenance therapy as compared to opioid‐naïve individuals . Interestingly, insomnia and short sleep duration have also been found to be significant risk factors for onset of chronic pain, with poor sleep increasing anxiety and hyperalgesia to heat, blunt pressure, cold, and pinprick stimuli . These findings indicate that there may be a bidirectional relationship between sleep and pain, with those who have trouble sleeping being more likely to be in pain and those who are in pain being less able to sleep.…”
