Relation of adenoma to carcinoma in the gallbladder

Abstract: In order to clarify the relation of adenoma to carcinoma in the gallbladder, histopathologic examination was made on surgical specimens of 1605 cholecystectomies. Among them, 11 benign adenomas, seven adenomas with malignant change, and 79 invasive carcinomas were found. All of the benign adenomas were 12 mm or less in diameter (average diameter, 5.5 ± 3.1 mm), while the adenomas having cancerous foci were 12 mm or more in diameter (average diameter, 17.6 ± 4.4 mm). Most invasive carcinomas were more than 30 m… Show more

“…On the other hand, adenomyomatosis of the gallbladder is generally considered to carry no risk of malignant transformation, although a few cases have been reported where adenocarcinoma was associated with adenomyomatosis (Katoh et al, 1988;Ootani et al, 1992). Adenoma has been characterized as a precursor of GBC (Kozuka et al, 1982). We immunostained sections of pre-cancerous lesions together with sections from positive control specimens affixed to the same glass slides to minimize the chance of a false-negative result.…”

“…3) [1,3,33,34]. The sequence of hyperplasia-dysplasia-carcinoma, regarded as the prevailing mechanism underlying the development of biliary tract cancer in PBM, is thought to differ from both the adenoma-carcinoma sequence and de novo carcinogenesis associated with biliary tract cancer in the population without PBM [35][36][37].…”

“…Subsequently, the atypical epithelium undergoes increased stratification leading to the high-grade premalignant lesion called carcinoma in situ (27,28). Besides this most frequent evolution, a small number of gallbladder carcinomas seems to evolve from preexisting adenomas (29).…”
