Related Markers for the Precision Diagnosis of Complex Appendicitis in Children

Abstract: Acute appendicitis is the most common surgical emergency in children. Despite the high incidence rate of appendicitis, it is sometimes misdiagnosed or missed. Complex appendicitis (CA) in children is characterized by a critical condition, several complications, and high mortality. Precision distinguishing between simple appendicitis and CA correctly is key to choosing appropriate treatment. A safe, cheap, rapid, extensive and accurate diagnostic marker of appendicitis will be of great significance for emergenc… Show more

“…However, the identification of patients with non-complicated AA is still challenging. As CT scans involve radiation and are generally avoided for pediatric diagnostics [ 74 ], the prediction of complicated AA typically relies on patient history, clinical findings, laboratory values such as WBC count, granulocyte count, CRP concentration, and ultrasound [ 75 , 76 ]. Many efforts have been made to find new biomarkers to help differentiate between complicated and uncomplicated appendicitis [ 77 ].…”

“…Classically, the rapid diagnosis of acute appendicitis is commonly based on a brief detailed history, a focused physical examination, and directed laboratory findings that consist of clinical diagnostic scoring systems [ 12 ]. The Alvarado score, appendicitis inflammatory response score, and the pediatric appendicitis score have been clinically applied for diagnosing appendicitis and may be helpful for clinical application [ 13 , 14 , 15 ]. Recently, laboratory markers in differentiation between uncomplicated and complicated appendicitis have been explored.…”

“…Esta distinción, de enorme relevancia terapéutica y pronóstica, se confirma con el estudio histopatológico de la pieza de apendicectomía, aunque su estratificación de sospecha se establece con la impresión intraoperatoria reportada por el Cirujano Pediátrico durante la intervención. La evidencia existente en la literatura científica en relación al papel discriminatorio de los biomarcadores descritos entre estas dos entidades es muy limitada [53].…”
