Relapsed Club Foot

Abstract: 1. An operation which can correct congenital club foot deformity in older children is described. 2. The operation is based on the assumption that the essential element in a complex tarsal deformity is a displacement of the navicular bone on the talus and that all other elements of the deformity are secondary and adaptive. 3. The results of the operation are noted and the implications of some of its effects are discussed.

“…Calcaneal osteotomy may correct heel varus (Dwyer IO63), calcaneocuboid joint resection and fusion may shorten a too long lateral foot ray (Evans 1961), an opening wedge osteotomy of the medial cuneiform bone may elongate a too short medial ray (Hofmann el al. 1984) and a residual adduction of the forefoot can be corrected by metatarsal osteotomies (Berman and Gartland 1971).…”

“…The commonest problems are the insufficient release of the posterolatera1 tether and the undercorrection of the reduction of the subtalar complex. In these cases a calcaneocuboid resection and fusion (Evans 1961) is sometimes neccssary to straighten the foot. I have no personal experience of the lengthening procedure of the medial ray described by Hofmann et al (1984).…”
