Relapse pattern and second-line treatment following multimodality treatment for malignant pleural mesothelioma

Abstract: OBJECTIVES: To analyse the relapse pattern and influence of second-line treatment after recurrence of malignant pleural mesothelioma (MPM) in patients who had previously undergone multimodality treatment. METHODS:Between September 1999 and December 2013, 136 patients underwent macroscopic complete resection (MCR) by extrapleural pneumonectomy after induction chemotherapy for MPM. We analysed 106 patients who presented with recurrent disease until October 2014. Data were retrieved from our mesothelioma database… Show more

“…Few studies have analyzed outcome after recurrence following multimodality therapy. Kostron et al published a cohort of 106 patients with recurrent MPM after treatment with induction chemotherapy followed by EPP ± adjuvant radiation . They reported a median postrecurrence survival (PRS) of 7 months (95% confidence interval 5–9 months).…”

“…In addition, a recent phase 2 trial of postoperative radiotherapy in MPM showed no survival benefit [ 7 ]. It is also reported that, despite some small improvements in local control of patients with postoperative radiation, regional relapse remains the predominant pattern of tumor recurrence after multimodality treatment [ 8 ].…”

“…Other papers report surgical treatment of relapse in general and include not only CWR, but also contralateral pleurectomy, resection of a mediastinal tumor, radical retroperitoneal resection, pectoral muscle resection, and upper lobe segmentectomy. [93][94][95] In addition, the number of patients receiving second-line surgery was low and ranged from 2 to 16. [93][94][95] For selected patients, curative CWR is an effective strategy with minimal morbidity and promising survival rates.…”
