Objective To explore the care experiences of spouses as long-term and primary caregivers for disabled older adults in China. Methods A descriptive phenomenological method was used in this study, as well as purposive and convenient sampling. Semi-structured interviews were conducted with 15 spousal caregivers in Guangdong, China, from March to December 2021. Interview audio-recordings were transcribed verbatim and data were analyzed using Colaizzi's phenomenological analysis method. Results We identified four themes from the data: spousal care motivation; sacrifices in caregiving; obstacles in caregiving; spousal caregivers’ positive experiences. Conclusions Spouses took responsibility for providing care for their disabled partners regardless of their willingness. They had positive experiences while providing care, but negative experiences were dominant, especially for spouses of severely disabled older adults. Spouses are always perfect in caregiving roles, although they may also need medical assistance. To prevent a decline in spousal caregivers’ quality of life and relieve their care burdens, health care providers should support them as soon as possible or offer formal care for disabled older adults. It is necessary to intervene considering disabled older adults and their spousal caregivers as a unit to empower their confidence in coping with life together.

The Delta variant (B.1.617.2) has dominated in many countries over the world. Its sudden outbreak in China has led the government to quickly carry out large-scale nucleic acid testing to curb its spread. This qualitative study aims to find the challenges based on empirical evidence from the perspectives of the different groups of people involved in the testing, and further explore possible strategies to improve the efficiency of large-scale nucleic acid testing. Using a phenomenological approach, we selected 35 participants (seven managers, eight health professionals, six community volunteers and 14 residents) by purposive sampling. The interviews were conducted by in-depth semi-structured interviews and the data were analyzed by Colaizzi’s seven-step method. Qualitative analysis revealed three main themes: unreasonable and unsafe testing points layout settings, human and medical resources challenges, and potential infection risk. From the different angles, participants all experienced challenges during large-scale nucleic acid testing, making positive planning and adequate preparation important parts of the smooth development of testing. Large-scale nucleic acid testing relies on the cooperation and efforts of all to support containment of the spread of the virus. Local governments should improve their ability to respond to and deal with public health emergencies.

Background Smart hospital at home(HaH) have become a potential solution to the medical service needs of patients at home, but little is known about their application effects in the disabled elderly. It is necessary to map HaH management models and describe their forms, content, strategies, and evaluation indicators. Methods A scoping review was conducted based on the framework from Arksey and O’Malley. PubMed，Web of Science，Wangfang，CNKI，VIP and OpenGrey were searched until June 30,2022. Mapping existing the smart HaH management for the disabled elderly, including form, content, strategy, and evaluation. Results There were a total of 8 included studies，which focused on the following issues: The forms of management covering: mobile medical and telemedicine;The content covering clinical consultation, establishment of patient health records, guidance on equipment use, and alarm management; Strategy covering the formation of multidisciplinary teams, clarification of team members' responsibilities, comprehensive assessment before bed admission, and dynamic grade assessment throughout the process; Evaluation covering patient health outcomes, smart medical dependence, patient satisfaction evaluation, management program compliance and feasibility; Conclusions Smart HaH management presents comprehensive, holistic, targeted, and dynamic, and is relatively consistent in terms of management form, content, and strategy characteristics, and has achieved positive application effects. However, there is still a lack of high-quality research data and standardized and unified management evaluation criteria, and there is an urgent need to conduct a multi-center, large-sample study of smart HaH management programs for the disabled elderly.
