Rehabilitation following reverse shoulder arthroplasty

Abstract: Background. Reverse shoulder arthroplasty (RSA) shows a growing number of implantations and is a valuable option to improve shoulder function and decrease pain. However, there is no consensus concerning the rehabilitation protocol following surgery.

“…Besides, after RSA, the stability and mobility of the glenohumeral joint become dependent on the deltoid [28]. As a result, there is a higher requirement for postoperative physiotherapy [10,43], which results in higher costs. Moreover, given the higher rate of multiple complications, and especially when the surgical cost of RSA and length of hospital stay are signi cantly higher, the use of RSA instead of TSA in this group of patients may not be justi ed.…”

“…(1)(2)(3)(4) However, despite orthopaedic acceptance, significant disagreement exists regarding rTSA rehabilitation guidelines following surgery. (5)(6)(7)(8)(9)(10)(11)(12) Traditionally, postoperative rehabilitation has centered on staged physiotherapy followed by early protective and restrictive joint immobilization.…”

“…As a consequence, conservative postoperative rehabilitation orthodoxy has recently been challenged by a select few. (6,7,10,20,21) These outliers adopted an accelerated rehabilitation approach, shortening post-surgical immobilization to a minimum of two weeks rather than the "agreed-upon" expert recommendation of four to six weeks. Coupled with early initiation of range of motion (ROM) training, their protocol sought to optimize and restore shoulder mobility, eliminate postoperative joint capsule adhesions, and scarring between muscle groups; complications commonly associated with prolonged and restricted shoulder immobilization.…”
