Regulation of autoimmune diabetes: Characteristics of non‐islet‐antigen specific therapies

Abstract: Summary Non-islet-antigen specific treatments have been shown to alter the natural history of insulin dependent diabetes in both the non-obese diabetic (NOD) mouse and in recently diagnosed patients. However, concerns have been raised regarding the possibility that non-islet-antigen specific therapy may trade cell mediated autoimmunity for antibody dependent autoimmunity. Female NOD mice at approximately 70 days of age were treated with the non-islet-antigen specific agents complete Freund's adjuvant (CFA) and… Show more

“…The repeated epicutanous application of the contact allergens gave a local harmless subclinical dermatitis. The application involved only the outside of the ears, which is in contrast to the systemic and immunological unspecific treatments with anti-CD3 [32] and complete Freund's adjuvant [33] or the systemic treatment with BCG vaccination [34].…”

“…Evidence indicates that Th1-mediated autoimmunity can be downregulated by infectious pathogens such as mycobacteria (13,16,22,34,38). Several regulatory pathways have been suggested to mediate this downregulation.…”

“…Also, both CFA and Mycobacterium bovis bacillus Calmette-Guérin (BCG) vaccine were shown to block diabetes in nonobese diabetic (NOD) mice (16,29,37). BCG vaccine given to newly diagnosed human diabetics led to clinical remissions of various lengths in 11 of 17 cases (29).…”

“…21 the apparent ineffectiveness of BCG for the prevention of t1Dm in humans might be explained by its much weaker immunostimulatory effect than FCa. 22 infection with a live, attenuated strain of Salmonella typhimurium was shown to prevent autoimmune diabetes in the noD mouse. 23 the protective mechanism was suggested to involve altered chemokine expression and priming and trafficking of t cells in the infected mice, leading to downregulation of auto reactive t cells.…”
