Abstract. Immigrants may be carriers of infectious diseases because of the prevalence of these diseases in their country of origin, exposure during migration, or conditions during resettlement, with this prevalence being particularly high in sub-Saharan Africans. We performed a retrospective review of 180 sub-Saharan immigrants screened for infectious diseases at an International Health Center from January 2009 to December 2012. At least one pathogenic infectious disease was diagnosed in 72.8% patients: 60.6% latent tuberculosis infection, 36.8% intestinal parasites (intestinal protozoa or helminths), 28.1% helminths, 14.8% hepatitis B surface antigen positive, 1.2% anti-hepatitis C virus positive, 1.2% human immunodeficiency virus-positive, and 1.2% malaria. Coinfections were present in 28.4%. There was significant association between eosinophilia (absolute count or percentage) or hyper-IgE and the presence of helminths (P < 0.001). Relative eosinophilia and hyper-IgE were better indicators of helminth infection than absolute eosinophilia, particularly for schistosomiasis and strongyloidiasis. We found a high prevalence of infectious diseases in sub-Saharan immigrants, which could lead to severe health problems (in the absence of prompt treatment), representing a high cost to the public health system and possible transmission in the host country. Accurate screening and tailored protocols for infectious diseases are recommended in sub-Saharan immigrants.

The IgE-mediated immune system activation can be redirected to combat tumors. Mouse and human IgE have been shown to provide a potent adjuvant effect in antitumor vaccination, with a crucial role played by FcεRI. This effect results from T cell-mediated adaptive immune response. Modified vaccinia virus Ankara (MVA) has been used to infect IgE-loaded tumor cells. These results led to a shift toward a highly safe protocol employing membrane IgE (mIgE), thus eliminating any possible anaphylactogenicity caused by circulating IgE. Evidence that human mIgE and a truncated version lacking IgE Fabs (tmIgE) bind and activate FcεRI has been fundamental and forms the core of this report. Human tmIgE has been engineered into a recombinant MVA (rMVA-tmIgE), and the expression of tmIgE and its transport to the surface of rMVA-tmIgE–infected cells has been detected by Western blot and cytofluorimetry, respectively. FcεRI activation by tmIgE has been confirmed by the release of β-hexosaminidase in a cell-to-cell contact assay using human FcεRI-transfected RBL-SX38 cells. The rMVA-tmIgE antitumor vaccination strategy has been investigated in FcεRIα−/− human FcεRIα+ mice, with results indicating a level of protection comparable to that obtained using soluble human IgE tumor cell loading. The rMVA-tmIgE vector represents a device that suits safe IgE-based antitumor vaccines, harboring the possibility to couple tmIgE with other gene insertions that might enhance the antitumor effect, thus bringing the field closer to the clinics.

Latest estimates suggest 390 million infections of dengue occur each year, of which 100 million results in symptomatic disease. The new clinical classification proposed by WHO-TRD 2009 simplifies dengue fever (DF) into dengue (D) and severe dengue (SD) with an intermediary group of "dengue with warning signs" (DWS). Early, sensitive and specific diagnosis of dengue is needed for appropriate patient management as well as for early epidemic detection. Virological and serological markers in sera from dengue confirmed patients from Venezuela and Nicaragua were evaluated. A total of 232 sera collected in the acute (days 1 to 5 of fever onset) and convalescent (days 7 to 14) phases of illness were tested by Platelia NS1 antigen ELISA and IgM, IgA and IgE capture ELISA methods. NS1 was detected with highest OD values in the acute phase for primary and secondary cases. IgM was detected in primary and secondary groups but the highest OD figures were observed after defervescence day. IgA and IgE detection were almost absent for primary cases, contrary to secondary cases which showed higher values but after defervescence day. NS1 could be used as effective diagnostic marker in the acute phase of illness for primary and secondary dengue infections. IgM detection is still an invaluable tool for routine dengue diagnosis but delay. IgA detection could define secondary infection in early phase of illness. IgA/IgE could be evaluated as prognostic markers associated to SD cases.
