Regional Blood Flow, Contractility and Metabolism in Early Myocardial Infarction

Abstract: The effects of occlusion of the left anterior descending coronary artery on a variety of metabolic parameters was examined in both infarcted and non-infarcted areas of the dog heart. These included mitochondrial performance, glycolysis, in vitro contractility, and regional myocardial blood flow. Measurements were made at 1 and 3 h after onset of ischemia. Regional coronary blood flow was measured in infarcted, noninfarcted and borderline regions using radioactive microspheres. Blood flow through the ischemic a… Show more

“…Decreased systolic fiber shortening (1), increased anaerobic metabolites (2,3), microvascular damage (4,5), and decreased blood flow, especially in the subendocardium (6), are all characteristics of the ischemic or infarcted region. Decreased mitochondrial respiration (7), tissue Po2 (8), and increased 02 extraction as well as decreased 02 consumption (9,10) indicate alterations in 02 availability and demand. Recent work has shown substantial variability of small artery and vein saturations as well as heterogeneous regions of 02 extraction and consumption within the center of an ischemic zone caused by coronary artery ligation (11).…”

“…Emphasis was placed on mitochondrial function in the present study, since previous work has stressed that these organelles are sensitive indicators of myocardial damage under the conditions employed (14,(29)(30)(31)(32). The question arises whether the mitochondria isolated from the occluded region were in fact uniformly ischemic.…”

“…Mitochondrial dysfunction and ultrastructural subcellular damage have been previously identified as one of the first signs of cardiac ischemic injury, and this in turn affects infarcted cardiac wall motion [8,9]. Ischemic damage results in changes to mitochondrial respiration [9].…”

“…Ischemic damage results in changes to mitochondrial respiration [9]. Mitochondrial respiration can be measured in vitro from isolated mitochondria by incubating these subcellular fractions in an isotonic medium containing succinate and phosphate.…”
