Refractory Peritoneal Dialysis Peritonitis Due to Neisseria macacae: A Case Report and Review of the Literature

Abstract: The Gram-negative diplococcus Neisseria macacae is a commensal bacterium of the mucosal surfaces in humans. A 52-year-old woman receiving continuous ambulatory peritoneal dialysis was admitted because of abdominal pain and turbid peritoneal fluid. N. macacae was isolated from peritoneal fluid culture and showed susceptibility to ceftriaxone. Despite appropriate antibiotics, the peritonitis was refractory, leading to the removal of the peritoneal dialysis catheter. … Show more

“…Ceftriaxone – along with gentamycin – was also found to be successful in treating N. elongata peritonitis 8 . Intraperitoneal ceftazidime (2–3 weeks) was a successful agent in our patient’s treatment and another reported case of Neisseria subflava biovar perflava peritonitis 11 , both of which had a penicillin-resistant organism.…”

“…Multiple case reports showed the implication of traditionally known nonpathogenic/benign (nongonococcal and nonmeningococcal) Neisseria species 5 in serious medical conditions, for example, N. elongata and Neisseria mucosa endocarditis 6 , 7 . Peritonitis was also reported in such species, including N. elongata 8 , N. mucosa 5 , Neisseria cinerea 5 , 9 , Neisseria macacae 10 , and Neisseria subflava biovar perflava 11 .…”

“…Only one case of infectious endocarditis in an adult with N. macacae bacteremia was reported [2]. Previously, reports of N. macacae infection in humans include peritonitis in patients on peritoneal dialysis [5]. Addtionally, N. macacae has been isolated from the upper respiratory tract of neutropenic patients [4].…”
