Refractive, visual, and subjective quality of vision outcomes for very high myopia LASIK from − 10.00 to − 13.50 diopters

Abstract: Background: To evaluate laser-assisted in situ keratomileusis (LASIK) outcomes, subjective quality of vision (QoV) and patient satisfaction in eyes with very high myopia (VHM) above − 10.00 diopters (D). Methods: Consecutive myopic and myopic-astigmatism eyes with spherical equivalent (SEQ) ranging between − 10.00 to − 13.50 D underwent LASIK with the WaveLight® Allegretto Wave® Eye-Q 400 Hz excimer laser. Treatment accuracy, efficacy, safety, stability, cylinder vectors, and higher-order aberrations were eval… Show more

“…22,29,30 In high myopes evaluated 2 years after undergoing LASIK, UDVA was at least 20/20 in 50.9% of eyes, at least 20/25 in 80.7% of eyes, and at least 20/40 in 95.6% of eyes. 21 Myopic regression rates in our patients with hypermobile EDS were comparable or possibly even more favorable than those seen in the general population. Myopic regression can be caused by biomechanical changes in the cornea, epithelial hyperplasia, stromal remodeling, errors in treatment (imprecise preoperative refraction or not enough laser energy), development of cataract, or axial elongation.…”

“…With regard to the high myopia group in the current study (which we defined as a preoperative SE of −10.0 D or higher), the LASIK efficacy was poorer than for the other two groups, as only 26.5% of these eyes achieved a UCVA of ≥20/20, although 91.2% achieved a UCVA of ≥20/40. In previous reports on cases with more severe myopia, higher ablation was followed by more wound healing and myopic regression, and surgeons seemed to perform undercorrection when faced with limits imposed by corneal thickness [ 28 , 32 , 33 ].…”

“…Comparatively, in one study by Wallerstein et al, they operated on 114 eyes from 78 cases who experienced LASIK at the Canadian Institute of Refractive Surgery. After 24 months post-surgery, the scatterplot of the tried against actual refractive adjustment showed an expected technique with 51.7%, 71.9%, 84.2%, and 93.8% existing in ± 0.25 D, ± 0.50 D, ± 0.75 D, and ± 1.00 D, of SEQ aim [13]. Kanellopoulos and Asimellis operated on 160 eyes with high myopia ⩾ − 6.00 D, with postoperative average spherical equivalents at − 0.37, − 0.43, and − 0.25 D for the 3, 6, and 12 months, which was also predictable [14].…”
