Refractive keratoplasty for post‐graft astigmatism

Abstract: Refractive keratoplasty using modified relaxing incisions with compression sutures was performed on 33 patients with high post-keratoplasty astigmatism. The mean reduction in keratometric astigmatism was 8.38 dioptres from a preoperative mean of 12.21 dioptres (SD, 3.59) to 3.93 dioptres (SD, 2.13). All patients noted a marked functional improvement. Two patients required reoperation. One patient required resuturing. Our modification of undercutting the relaxing incisions greatly increases the range of astigma… Show more

“…3,[5][6][7][8][9][10][11] Several methods to correct these errors have been suggested, and the results have been variable. [17][18][19][20][21][22][23][24] We present a large series of In our study, there was a 60% decrease in refractive cylinder at 12 months and an overall reduction in SE in 83.3% of patients with low refractive cylinder. Although patients with high refractive cylinder also had a decrease in cylinder and SE, the reduction was not as pronounced (40.9% and 41.5% respectively).…”

“…16 Refractive surgical procedures, such as arcuate incisions, resuturing, retrephination, wedge excisions, and Ruiz-type techniques, have been used to treat post-graft astigmatism. [17][18][19][20][21][22][23][24] In general, they correct various amounts of astigmatism but with limited predictability and without allowing treatment of anisometropia.…”

“…Astigmatism following penetrating keratoplasty is a wellrecognized problem that reduces the function of a clear corneal graft.l.2 Although every attempt is made to reduce 'sutures in' astigmatism by meticulous surgery under keratometric control and suture manipulation in the early postoperative period, the final astigmatic result when the sutures are removed is still unpredictable. 3,4 We report an unusual case of localized corneal scarring and irregular astigmatism that developed 2 years after treatment of post-graft astigmatism with photorefractive keratectomy and arcuate incisions. Surgical excision of the scar restored visual acuity to 6/12 unaided.…”
