Refining Prognosis in Chemoembolization for Hepatocellular Carcinoma: Immunonutrition and Liver Function

Abstract: A combination of albumin-bilirubin (ALBI) grading and the Prognostic Nutritional Index (PNI) was identified recently as a highly predictive tool for patients with hepatocellular carcinoma (HCC) undergoing tumor ablation. The present study evaluated this combination in patients undergoing transarterial chemoembolization (TACE). Between 2010 and 2020, 280 treatment-naïve patients were retrospectively identified. The influence of ALBI grade, PNI and the novel ALBI-PNI on the median overall survival (OS) was asses… Show more

“…A previous study showed that the combination of the PNI, an immunonutritive scoring system, and the ALBI grade was a promising stratification tool for evaluating patients with HCC for remnant liver function and immunonutritive status [ 45 ]. In a recent study, we showed that the so-called ALBI-PNI was feasible, particularly among patients undergoing TACE [ 46 ]. Thus, our future research effort could include the integration of CRP as a relevant prognostic factor in the ALBI-PNI system.…”

“…Thus, current attempts try to include novel risk factors to improve the predictive performance of risk scoring systems. In particular, the knowledge on the interplay of tumor development and progression as well as immune response and inflammatory reaction has been growing continuously [55][56][57][58][59]. Especially the role of neutrophils for HCC development and progression on the tumor microenvironment as well as in systemic reaction is under investigation.…”

“…With the expansion in available therapies, prognostic factors play an increasingly important role, among which the most relevant in patients with HCC are those indicative of liver function, such as Child-Pugh (CP) class and albumin-bilirubin (ALBI) grade [6][7][8][9][10]. Recently, there are several evidences in the literature about the fundamental role of both the state of chronic inflammation and the nutritional state in the prognosis of HCC patients [11][12][13][14][15][16][17][18].…”
