Referrals to the Wills Eye Institute Cornea Service after laser in situ keratomileusis: Reasons for patient dissatisfaction

Abstract: Most patients who came for consultation were referred by a doctor other than their LASIK surgeon. Poor distance vision, dry eye, redness/pain, and glare and halos were the most common chief complaints and dry eye or blepharitis, irregular astigmatism, and epithelial ingrowth, the most common diagnoses.

“…The total number of dissatisfied participants in the PROWL studies was too small to assess associations with other factors. Similar to other studies, 9,11,12,21,42,48 we found that lower satisfaction with vision and surgery was often associated with worse dry eye and visual symptom scores and the use of corrective lenses. To better understand which patients are more likely to be dissatisfied postoperatively, a large observational study, including participants with long-term follow-up after LASIK surgery, would be necessary to accurately estimate the prevalence and find useful predictors for these perceptions.…”

“…However, exploratory analyses suggested that dry eye symptoms were associated with visual Dry eye is one of the most common complications reported after LASIK surgery. 7,9,11,[35][36][37][38] Using the OSDI, with its limitations, 39 our findings for dry eye symptoms were incon- All correlations were performed using Spearman rank-order correlation coefficients (satisfaction score vs OSDI Score; all P < .001). Each panel shows analyses with satisfaction scores for 1 of 2 types of satisfaction: satisfaction with vision (A) and satisfaction with surgery (B).…”

“…[22][23][24] The regression analysis between selected subsets in the IER QOL scale and objective clinical data showed a significant association between the change in satisfaction post-LASIK and preoperative satisfaction, postoperative frequency of disturbing visual and ocular symptoms, postoperative MSE, and postoperative scotopic HCVA. Patients dissatisfied with their preoperative optical correction showed higher levels of change in satisfaction if a reduction in the frequency of disturbing symptoms, low levels of post MSE, and good levels of scotopic VA were achieved after LASIK.…”

“…3 Dry eye symptoms, dissatisfaction with visual outcomes, and burdensome visual symptoms have been reported following LASIK. 4,5 Given the high volume of surgery performed and patient concerns voiced at a public meeting, it is important to evaluate the effect of surgically induced changes on patientreported outcomes (PROs) of LASIK: eye-related symptoms, daily functioning and well-being (health-related quality of life [HRQOL]), and patient satisfaction. [6][7][8] A better understanding of PROs after LASIK may allow eye care professionals and patients to make more educated decisions.…”
