PurposeThe purpose of this study was to evaluate the safety and efficacy of microinvasive glaucoma surgery (MIGS) with 360° ab-interno trabeculotomy using the TRAB360 device as a stand-alone procedure in patients with refractory primary open-angle glaucoma (POAG) and preoperative IOP ≥18 mmHg.SettingThis study evaluated patients treated in a tertiary-referral clinical practice setting.DesignThis study is a retrospective analysis of 81 eyes.MethodsPatients with refractory open-angle glaucoma underwent stand-alone 360° ab-interno trabeculotomy using the TRAB360 device. Effectiveness was determined by reduction in medicated IOP and the use of medications from baseline. Safety was determined by the rate of adverse events and secondary surgical interventions. The time points assessed were baseline and postoperative day 1, week 1, and months 1, 3, 6, and 12. A subgroup analysis was performed on eyes with medicated preoperative IOP values of ≥25 mmHg.ResultsThe reductions in IOP from 1 day to 12 months postoperatively were statistically significant compared to baseline values. The mean reduction in IOP at 12 months was 7.3±6.7 mmHg from baseline. At 12 months, 59% eyes achieved ≥20% reduction in IOP and IOP <18 mmHg with the same or fewer numbers of IOP-lowering medications compared with those at baseline. The mean number of IOP-lowering medications was reduced from 1.7±1.3 at baseline to 1.1±1.0 at 12 months. At 12 months, 67% of eyes with preoperative IOP values of ≥25 mmHg achieved ≥20% reduction in IOP and IOP <18 mmHg with the same or fewer numbers of IOP-lowering medications compared with those at baseline. The most common adverse event for all eyes was mild, transient hyphema (57 eyes). During the first year after the procedure, 20 (25%) eyes were considered failures since they required reinterventions.ConclusionTrabeculotomy using the TRAB360 device resulted in significant IOP reductions up to 1 year with a favorable safety profile. The device is an effective stand-alone MIGS procedure for patients with refractory POAG.

Purpose. We aimed to investigate squeezing force and dispensing time interval of the first and second eye drops among a variety of eyedropper bottles and to clarify associated factors within these parameters. Methods. A total of 87 eyedropper bottles were involved in this study. We developed a squeezing force measuring system consisting of a syringe pump, digital force gauge, and custom-made test stand to measure the squeezing force and dispensing interval. The eyedropper bottle was housed in the system vertically, and measurements were repeated five times. We investigated the differences in squeezing force and dispensing interval by categories, including those that targeted ocular disease, brand or generic eyedroppers, shapes of eyedropper bottles, and the presence of a membrane filter inside the tip of the eyedropper bottle. Results. The mean squeezing forces of the first drop and the second drops were 8.3 ± 3.0 N and 10.4 ± 3.2 N, respectively. Both squeezing forces had a wide variation of more than threefold. A mean interval between the first and second drops was 3.1 ± 1.2 sec with a maximum difference of more than sevenfold. Round shapes of eyedropper bottles and the presence of filter membranes significantly increased the squeezing force. Conclusions. The squeezing force of eyedropper bottles was very wide, which may affect proper eye drop instillation. Unification of eyedropper bottles or developing mechanical aids may be useful for proper eye drop treatment.

Background: The evaluation of ab interno trabeculectomy, referred to as trabectome®, among Japanese patients is insufficient. Subjects and methods: Japanese patients who underwent trabectome® at the University of Yamanashi Hospital were included. The investigated parameters were intraocular pressure (IOP), best corrected visual acuity, glaucoma medications, visual field, and corneal endothelial cell density. The success rate and its associated factors were investigated. Results: A total of 250 eyes from 197 patients were enrolled. The trabectome® significantly reduced IOP and glaucoma medications up to 48 months. Concomitant cataract extraction enhanced the reduction in IOP and glaucoma medications up to 42 months. At 36 months postoperatively, 40.8% satisfied IOP of the same or less than 18 mmHg or more than a 20% IOP reduction with the same or less use of glaucoma medications as preoperatively. Preoperative IOP and combined cataract extraction were significantly associated with the success rate. The trabectome® alone did not show a significant reduction in corneal endothelial cells. Eyes with postoperative transient IOP elevation and removal of anterior chamber hemorrhage were 11.2% and 1.2%, respectively. Twenty-four eyes (9.6%) underwent additional glaucoma surgeries. Conclusions: The trabectome® could be considered an effective and safe surgery. Compared to trabectome® alone, combined cataract surgery was superior in lowering IOP and reducing glaucoma medications.
