Reduction in Neural-Tube Defects After Folic Acid Fortification in Canada

Abstract: D espite advancements in medicine, preterm infants continue to experience high neonatal mortality and morbidity, with many having lifelong neurodevelopmental deficits, such as cerebral palsy (CP) and impaired cognitive abilities. The risk of impairment increases with decreasing gestational age at birth and many studies of short-term and long-term outcomes have focused on infants born at <33-week gestation. There are few data, however, on neonates born at 33 or 34 weeks. Issues include optimal places for delive… Show more

“…This country not only fortifies wheat flour but also maize flour, cow's milk and rice (28) . Other countries showed NTD reductions of about 50 % (16,20,21,57) and similar reductions in spina bifida (21,25,38) and anencephaly (16,35,39) . The results varied by country and depended on the integrity and quality of the country's surveillance system.…”

“…For example, in Norway, the country with the highest rate of facial clefts in Europe, the fortification of foods with FA has not been allowed (17) . In contrast, flour and other grain products are being fortified at a minimum level of 0.14 mg FA/100 g cereal grain in Canada and the USA and a decline in NTD was observed as a result of that fortification (18,19) . However, the fortification level should be enough to provide a daily intake of 0?6 to 0?8 mg FA in order to attain the highest benefits (20) .…”

“…Several countries have introduced mandatory folic acid fortification to reduce NTD rates (4)(5)(6) . The reduction achieved in these countries has depended on the background rate of NTD; the higher the background rate the greater the reduction (6) .…”
