Reducing the Number of Unnecessary Percutaneous Biopsies: The Role of Second Opinion by Expert Breast Center Radiologists

Abstract: Background/Aim: The research objectives of this study were the estimation of the number of misdiagnosed breast lesions by non-expert-center-breast-radiologists (NEBR) and the investigation of the discordant rate (DR) calculated between initial and second opinion. Moreover, this study evaluated the impact of second opinion and the factors associated with DR. Materials and Methods: A total of 399 patients were sent to our Tertiary Breast Cancer (BC) Center to perform fine needle aspiration/core needle biopsy (FN… Show more

“…admitted to percutaneous procedure [as core needle biopsy/vacuum assisted biopsy (CNB/VAB)] for new SBL were selected for this analysis. The main inclusion criterion for retrospective enrolment was a complete previous imaging evaluation of the breast as well as new SBL classified as BI-RADS ≥4, according to breast imaging reports and data system lexicon (BI-RADS) (12), after revaluation by our Breast Expert Radiologists (13). Moreover, when indicated, our patients went through an 8-gauge Vacuum assisted biopsy to reduce the need for further treatment in case of benign lesions (14)(15)(16).…”

“…To avoid any detrimental effect on the clinical outcome, we routinely offer a preoperative psychological evaluation for any breast cancer patient. In addition to reduced patient mortality, treatment in a centralized tertiary facility reduces the number of unnecessary interventions (35), reduces the hospitalization of patients (36-38), and provides a lower rate of complications during reconstruction surgery (1) or innovative approach to reduce the impact of surgery (36, 39). In our opinion, tertiary breast cancer facility could produce a higher medical standard for patients and could provide a better medical treatment for breast pathology.…”
