Objective To evaluate the phenotypic features of the masticatory biomechanics in atypical subjects with Down syndrome (DS). Its influence was analysed on sleep disorders, body adiposity and its risks, and some physicochemical properties of saliva. Methods Seventy subjects were enrolled to assess masticatory biomechanical function and divided into two groups: DS and control groups. Electrical activities of the masseter and temporal muscles (at rest and in maximum voluntary clench‐MVC), maximum bite force‐MBF and maximum mouth opening‐MMO were investigated. Among the atypical subjects, just 24 participants underwent the anthropometry, the polysomnography II and the saliva testing (salivary flow rate‐SFR, buffer capacity‐BC and salivary cortisol levels, morning/SC‐AM and night/SC‐PM). Results MVC and MBF values showed high statistical significance in the control group (P < .001) than in the DS group of 35. MMO values were slightly increased in the DS group in relation to the control group. Overweight and obesity were found in both genders. Atypical women showed higher risk to develop cardiovascular‐metabolic diseases than in atypical men. OSA severe was 20% for atypical women and 42.8% for atypical men, whereas snoring index was present in all genders. SFR was reduced in 100% of atypical subjects (hyposalivation in 10% women and 28.5% men). Furthermore, 100% BC, 66.6% SC‐AM and 91.6% SC‐PM showed normal patterns. Conclusion Masseter and temporal muscle hypotonia was found in all atypical subjects with DS. This muscle dysfunction strongly was related to overweight/obesity, risks for development of cardiovascular/metabolic diseases, OSA severity, successive snoring episodes and salivary flow reduction in DS.

Síndrome de Down (SD), é uma condição genética em que ocorre a formação de um cromossomo a mais. Palato ogival, macroglossia, língua fissurada, prevalência reduzida de cáries e aumentada de doença periodontal (DP) são as principais manifestações orais. Através de uma revisão narrativa verificar a relação da DP com a SD. Foram selecionados estudos transversais, longitudinais e revisões sistemáticas, em português e inglês, indexados nas bases de dados do portal de periódico CAPES, PubMed e Google Acadêmico; foram utilizados os descritores “periodontite”, “SD” e “DP”. Caso clínico, relato de caso e artigos de opinião foram excluídos. Primeiro foi realizado uma triagem pelo título seguida de leitura dos resumos, após constatar relação com o tema, os artigos foram vistos na íntegra por um único pesquisador. Indivíduos com comprometimento intelectual, muitas vezes, apresentam higienização bucal deficiente. Entretanto, apenas este fato não é capaz de explicar a razão do sindrômico ser mais susceptível à DP. Indivíduos com SD apresentam alterações no sistema imune, embora o número de células imunes seja normal, suas funções de quimiotaxia e fagocitose são diminuídas juntamente com resposta imunológica de IgA e IgG. , Sugere-se que a prevalência da DP em indivíduos com SD seja devido a deficiência do sistema imune e não somente à má higienização. Conhecer o processo imunogenético dos indivíduos com SD poderá trazer soluções mais eficientes para o tratamento odontológico destes indivíduos e contribuir com sua qualidade de vida.

BackgroundPeriodontal diseases (PDs) have been documented to be significantly more prevalent and severe in patients with Down syndrome (DS). Different immunological and microbiological factors contributed to predisposing these patients to progressive and recurrent PDs.AimThe aim of this review was to investigate the altered immunological responses and oral microbiota disorders as well as focus on adjunctive non‐surgical methods for the treatment of PDs and its applicability in patients with DS.Material and MethodsA literature review was conducted addressing the following topics: (1) the altered immunological responses, (2) orofacial disorders related to DS patients, (3) oral microbiota changing, and (4) adjunctive non‐surgical treatment and its efficacy in patients with DS.ResultsDue to the early onset of PDs in children with DS, the need for prompt and effective treatment in these patients is essential.Discussion and ConclusionSo, investigating underlying factors may open a new window to better understand the pathology of PDs in DS people and thus, find better strategies for treatment in such group. Although non‐surgical treatments such as photodynamic therapy and probiotic consumption represented acceptable outcomes in different examined patients without DS, data about the application of these convenience and no need for local anesthesia methods in patients with DS is limited.
