Redefining Diagnostic Symptoms of Depression Using Rasch Analysis: Testing an Item Bank Suitable for DSM-V and Computer Adaptive Testing

Abstract: An item bank of 17 items may be useful in computer adaptive testing and nine or even four items may be used to develop a simplified screening tool for DSM-V major depressive disorder (MDD). Further examination of this item bank should be conducted in different cultural settings.

“…First, we must consider that in many contexts psychiatric diagnostic is based on a checklist symptom profile, where every symptom is given the same weight to diagnosis (except mood and anhedonia, which are considered necessary symptoms). Understanding the symptoms as a clinical marker of severity may suggest that some of them (e.g., guilt feelings and psychomotor retardation) should have a higher weight on the diagnosis of MDD, in spite of other symptoms that may be not part of the same underlying unidimensional construct [57]. Second, this could be the start point to understand how the severity of melancholia can be related to different brain areas, since different MDD symptoms does not seem to affect only one region of the brain [58].…”

“…4 According to the Diagnostic and Statistical Manual of Mental Disorders, 5th Edition, the criteria to diagnose MDD is five of these symptoms for 2 weeks. 5 The investigations into the neurobiology of MDD have focused on different neurotransmitter systems. Since reserpine, a known antihypertension medication that depletes monoamines in the nerve system, 6 can cause depressive symptoms, a monoamine hypothesis proposed that depression patients probably have low levels of monoamine neurotransmitters, epinephrine and serotonin.…”

“…In the current international climate, how pleasing it is to see an attractive piece of psychometric research come out of Iraqi Kurdistan, achieved by psychiatrists there in collaboration with UK colleagues. Mitchell et al [7] used the Rasch model to construct a parsimonious tool for detecting depressive disorder. The Rasch model is the simplest model in Item Response Theory, having only one parameter, the threshold.…”
