Recursive partition analysis of peritoneal and systemic recurrence in patients with gastric cancer who underwent D2 gastrectomy: Implications for neoadjuvant therapy consideration

Abstract: We proposed risk stratification models of peritoneal and systemic recurrence in patients undergoing D2-gastrectomy. This classification could be used for stratification protocols in future studies evaluating adjuvant therapies such as preoperative chemoradiotherapy. J. Surg. “…Additionally, the staining intensity was semi-quantitatively scored as 0 (negative), 1 (weak), 2 (intermediate), or 3 (strong). The final immunoreactivity score was defined as the sum of both parameters, and the samples were grouped as having negative (0), weak ( 1 2 ), moderate ( 3 ), or strong ( 4 – 6 ) staining. For statistical purposes, only the final immunoreactivity scores of the moderate and strong groups were considered positive, and the other final scores were considered negative.…”

“…Due to the lack of effective biomarkers for an early diagnosis, the 5-year survival rate of patients with advanced GC is only 5–15% ( 4 , 5 ). Surgical resection may be the only hope for advanced-stage GC patients, but has a high rate of recurrence ( 6 , 7 ). Therefore, it is imperative to explore novel biomarkers and therapeutic targets that may be helpful in developing targeted therapies for GC.…”

“…These factors contribute to a poor prognosis of gastric cancer due to a lack of an effective treatment regimen and correlation data statistics shows the five‐year survival rate of gastric cancer is only 24–25% . Despite recent progress in gastrectomy, chemotherapy and radiotherapy, a large number of all advanced stage gastric cancer patients die of recurrence, even after undergoing curative gastrectomy and adjuvant treatment . To address these issues and to aid in the development of novel targeted therapies for gastric cancer, it is imperative to elucidate the regulatory mechanisms and the molecular alterations modulating tumorigenesis.…”

“…For locally advanced gastric cancer, the locoregional recurrence rate and distant metastasis rate are still high after D2 radical resection [3][4], leading to poor prognosis of these patients. Therefore, adjuvant chemotherapy after radical surgery is currently recommended for all locally advanced gastric cancer [5].…”
