Recurrent Implantation Failure: Reality or a Statistical Mirage?: Consensus Statement From the July 1, 2022 Lugano Workshop on Recurrent Implantation Failure

Abstract: Despite advancements in assisted reproductive technologies over the past few decades, a 35% failure rate is still observed for unexplained reasons when transferring euploid embryos to an anatomically normal uterus. Recurrent implantation failure (RIF) is defined based on cumulative ART cycles failing; however, there is an absence in consensus on the diagnostic criteria, which leads to the risk of overdiagnosing and overtreating the condition. This consensus statement by the Lugano RIF Workshop aimed … Show more

“…Notwithstanding tremendous improvement in in vitro fertilization (IVF) for decades, recurrent implantation failure (RIF) is still a challenging and poorly defined clinical phenomenon affecting 5%−10% patients undergoing IVF worldwide ( 1 , 2 ). To date, RIF has no universally accepted definition, considering the number of unsuccessful IVF cycles, the number of embryos transferred, the quality of embryos, embryo stage, and maternal age ( 1 , 3 , 4 ).…”

“…3 Meanwhile, ASRM suggests that RIF diagnosis should focus on failure to achieve "sustained" implantation (defined as a gestational sac identified on ultrasound). 4 RPL is a heterogeneous condition with multiple non-exclusive etiologies and contributing risk factors, including genetic, endocrine, anatomic, thrombophilic, environmental, and immunological factors.…”

“…Based on recent data, ESHRE now proposes that RIF describes the scenario in which the transfer of embryos considered viable has failed to result in a positive pregnancy test sufficiently often in a specific patient to warrant consideration of further investigations and/or interventions 3 . Meanwhile, ASRM suggests that RIF diagnosis should focus on failure to achieve “sustained” implantation (defined as a gestational sac identified on ultrasound) 4 …”
